Position Summary

We are seeking a dynamic and relationship-driven Community Liaison to join our growing team in Gillette, WY. This role is responsible for developing and maintaining key referral relationships, increasing census growth, and serving as a trusted resource to healthcare providers and the community.

The ideal candidate is a motivated self-starter who thrives in a fast-paced environment and is passionate about connecting patients with the care they need.

Key Responsibilities

Develop and manage relationships with physicians, hospitals, case managers, skilled nursing facilities, and other referral sources
Drive patient census growth through strategic outreach and territory planning
Educate referral partners on Healing Hearts’ services, programs, and clinical capabilities
Identify new business opportunities and expand market presence
Coordinate with internal clinical teams to ensure seamless patient admissions
Track and report referral activity, trends, and outcomes
Represent the organization at community events, health fairs, and networking opportunities
Maintain compliance with all healthcare regulations and company policies

Qualifications

Bachelor’s degree in healthcare, business, marketing, or related field preferred
Minimum of 1–2 years of experience in healthcare sales, home health, hospice, or related field strongly preferred
Proven ability to build and maintain professional relationships
Strong communication, presentation, and organizational skills
Self-motivated with the ability to work independently and manage a territory
Knowledge of home health services and referral processes preferred
Valid driver’s license and reliable transportation
