Full Job Description

POSITION: Clinical Pharmacist

SALARY: Depending on Experience

STATUS: Permanent Full-Time

POSITION SUMMARY:

Performs professional pharmaceutical work in an ambulatory care setting. Daily functions consist of processing, verifying, and dispensing prescription medications while practicing cost saving measures. Pharmacists heavily collaborate with medical providers, nurses, and laboratory staff to benefit patient outcomes.

DUTIES AND RESPONSIBILITIES

Preforms order entry, verifies order is entered and filled correctly, ensures prescription is accurately prepared for patient pick up. Counsels' patients or family members on the proper use, storage, potential side effects, and disposal of all medications dispensed.
Orders and interprets laboratory tests per collaborative practice agreement.
Addresses problem prescriptions in a timely manner prior to processing by addressing with the medical provider.
Uses clinical judgement to extend refills or provide partial prescriptions until next scheduled appointment under the provider's name per facility collaborative practice agreement.
Proactively assesses prescribing patterns to promote the safe use of appropriate medication therapy.
Participates in pharmacy managed clinics such as diabetes management, anticoagulation, smoking cessation within the scope of practice outlined in the collaborative practice agreement.
Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics.
Monitors for appropriateness, side effects, and clinical outcomes, and advises prescribers and nursing staff as appropriate.
Provides patient-specific therapeutic drug monitoring, and communicates relevant findings and or recommendations to other providers both orally and in writing.
Conducts prospective review and intervention to assess drug safety and efficacy. Provide medication counseling to patients and educates them on pertinent information and outcome.
Documents interviews with complete medication histories.
Prescribes medications as allowed per state and federal law.
Dispenses prescriptions and over the counter drugs without the patient seeing the doctor to treat minor ailments.
Serves as a drug information resource.
Reviews charts, evaluates pertinent laboratory data, drug interactions, monitors for adverse drug effects and screens for allergies.
Performs medication reconciliation during clinical interactions during patient care.
Completes all assigned/required documentation or other assignments without intentional leave of work for others
Reports metrics and measures in an accurate and timely manner
Participates with collateral duties that benefit the operational success of the pharmacy.
Demonstrates willingness to perform duties as deemed necessary by the supervisor.
Prioritizes all duties appropriately to ensure efficiency.
Actively participates in committees as assigned to promote pharmacy participation.
Collaborates with other healthcare professionals and/or administration to develop protocols related to pharmacy
Reviews and updates patient's medication profile, medication or dosage changes, allergy information, updates problem list, and monitors for compliance or potential abuse.
Documents clinical activities and interventions with respect to outcomes.
Reports adverse drug events, near misses, and medication errors.
Recommends formulary alternatives as needed or requested.
Provides leadership in identifying and developing cost avoidance strategies.
Ensures that medication related issues are resolved as soon as possible to provide optimal care.
Conducts prescription drug monitoring program review for every controlled substance prior to processing and document if there are findings or not in chart.
Selects and administers appropriate vaccines to healthy adult (18 and older) patients.
Responsible for maintaining professional competency by keeping abreast of current medical and pharmaceutical literature, new developments in pharmacy practice and new drugs and treatment plans and applies knowledge in daily duties.
Provides education to patients, health care providers, other pharmacy staff/students, and support staff regarding medication use and or preparation.
Conducts just in time and on the job training as needed.
Performs other duties as assigned.

QUALIFICATIONS:

Graduation from an accredited college or university with a degree in pharmacy.
Licensure as a registered pharmacist from a state, the District of Columbia, or a territory of the United States
CPR Certification or ability to obtain within six months of hire.
Certification in Pharmacy-Based Immunization Delivery or ability to obtain within six months of hire
Communication: Excellent verbal and written communication skills
Computer Skills: Word processing, spreadsheets, pharmacy software, and databases

Preference will be given to qualified Eastern Shoshone tribal members, then other qualified federally recognized Indian tribal members and then other qualified candidates.

Applicants must submit a copy of Tribal Enrollment card or CIB for Indian Preference.
Veterans who meet the minimum qualifications and provide documentation of an honorable discharge (DD214) from any branch of military service are entitled to receive preference points during the interview process.

Any offer of employment is contingent upon a negative drug test, reference checks and background check. Refusal to undergo required testing or testing positive will render the applicant ineligible for employment.
