Full Job Description

JOB SUMMARY

Works under the supervision of the Pharmacy Manager or Registered Pharmacist, and according to the standard procedures, compounds and dispenses medications and pharmaceutical supplies upon receiving prescriptions from physicians or other qualified prescribers; checks Cart, IV batch, Pyxis and crash cart fills; prepares IV admixtures and oral special orders, provides drug information to other healthcare professionals, completes projects for pharmacy department.

ESSENTIAL JOB FUNCTIONS:

Medication reconciliation and counseling.

Written progress notes in patient chart under supervision of a pharmacist.

After completion of aseptic technique and high risk medication training and demonstration of competency, compounds IV admixtures.

Participation in interdisciplinary team rounding.

Medication Discharge list review and patient education.

After completion of orientation and demonstration of competency, entry of prescribed dosage and/or verification of CPOE orders under direct supervision of a pharmacist.

Medication renal dosing and pharmacy medication consults under supervision of a pharmacist.

Performs patient charging functions:

When required, types labels for medications, as well as IV solutions in a neat and accurate manner.

Helps pharmacy technicians accurately enter into the computer all patient charges.

Accurately refills Pyxis machines and maintains proper inventory levels.

Accurately refills patient medication drawers.

IV preparation:

Correct drug and dose selected.

Correct type and volume of diluent used.

Correct and aseptic techniques used for preparing IV medication.

IV hood is properly cleaned.

All IV preparations are prepared in a timely manner.

Assures adequate stock and supplies are available in IV room to perform next day’s activities.

Disposal of outdated IVs and proper logging.

Successful completion of the aseptic technique certification checklist.

Ordering and inventory control:

Researches and explores all possible sources for unavailable pharmaceuticals.

Helps pharmacy technicians to keep all pharmacy personnel informed of back-ordered or unavailable pharmaceuticals.

Helps pharmacy technicians to unpack and store supplies, verify items received against invoices, check for price discrepancies, check off invoices and pass them on to the Manager assures all contract items are charged at contract amount, investigates shorts and searches for alternative source.

Utilizes generic equivalents whenever available.

Routinely handles returns of unused patient medication and returns to proper storage area.

Maintains work area in a neat and orderly manner at all times.

At regular intervals, conducts inspections of areas where pharmacy items are stocked. Removes any outdated drugs at that time.

Helps pharmacy technicians organize outdated or overstocked medications to ship back to the manufacturer or wholesaler for credit on a regular basis. Helps pharmacy technicians to assure that proper credit is received from the wholesalers and maintains a file on credit memos from the wholesalers.

Answers the telephone and consults with the on duty pharmacist, as necessary, about questions on medication orders, drug information, poison information or other clinical matters.

Demonstrates competency on a consistent basis in regards to weighing and measuring of liquids, powders, tablets or capsules for unit dosing purposes.

Maintains updated ordering information concerning phone numbers, location, etc. of different pharmaceutical companies for medications not ordered from the wholesaler.

Maintains proper records associated with prepackaging.

Helps pharmacy technicians maintain all stock level and order stickers for all formulary items.

Helps pharmacy technicians that the proper amount of chemo medication is ordered and on hand for next week’s chemo regimes.

Checks for outdates as necessary, both within the pharmacy department and outside the department.

Performs other duties as assigned or needed to meet the needs of the department / organization

EDUCATION/ EXPERIENCE/ LICENSE and CERTIFICATIONS

Enrolled in school of pharmacy program.

Certified Pharmacist Intern with Wyoming State Board of Pharmacy.

ADDITIONAL SKILLS

Able to communicate effectively in English, both verbally and in writing

Good interpersonal skills needed to work with staff and physicians

Strong written and verbal skills

Strong computer knowledge

Hand/eye coordination, fine and gross motor skills

Able to prioritize multiple details simultaneously, able to troubleshoot problems, analytical, interpretive skills

Requires higher level of short- and long-term mental faculties accompanied by long- and short-term memory

Specific demands not listed: Possible exposure to blood and or body fluids / infectious disease / hazardous waste requiring the use of Personal Protective Equipment. Exposure to odorous chemicals / specimens and Latex products.
