Full Job Description

Overview
Join our dedicated healthcare team as a Healthcare Assistant (HCA) or Certified Nursing Assistant (CNA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our patients. In this vital role, you will provide personalized support to individuals in our AFH. Your expertise in patient observation, medication administration, and assisting with Activities of Daily Living (ADLs) will ensure our residents receive the highest quality of care. This paid position offers an engaging opportunity to work with diverse populations, including seniors, individuals with developmental disabilities, and those requiring specialized dementia or hospice care.

HOURS- 2PM-10PM

DAYS FRIDAY THRU TUESDAY

Duties

Monitor and observe patients’ conditions regularly, documenting vital signs and medical changes accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
Assist residents with ADLs such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts and ensuring safety protocols are followed
Administer medications following prescribed protocols while maintaining strict compliance with HIPAA regulations to protect patient confidentiality
Support patients with specialized needs including Alzheimer’s care, memory care, dementia care, and hospice care by providing compassionate companionship and tailored assistance
Help with meal preparation and feeding for residents requiring assistance; ensure proper nutrition and hydration are maintained
Maintain accurate medical documentation and records of patient care activities in compliance with healthcare standards and facility policies

Experience

Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
Demonstrated knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities
Hands-on experience with vital signs monitoring, medication administration, catheter care, and dementia or Alzheimer’s care
Familiarity with EMR/EHR systems for documentation and patient record management
Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or working with individuals with developmental disabilities are a plus
Experience working with diverse populations including those with disabilities or requiring long-term care support
Ability to work independently or as part of a team in fast-paced environments while maintaining high standards of patient safety and confidentiality
