Full Job Description

Gateway Counseling Center is seeking an experienced, organized, and compassionate Intake Coordinator to manage the admissions process for individuals entering our OPWDD Day Habilitation and Day Treatment programs. This position serves as the primary point of contact for referral sources, Care Managers, families, and internal departments while ensuring a smooth, timely, and compliant enrollment process.

The ideal candidate has experience working within the OPWDD system and is familiar with the CHOICES portal, Medicaid verification, and electronic health records.

Key Responsibilities

Coordinate the intake process from referral through admission and orientation.
Review referral packets and obtain all documentation necessary for admission.
Communicate with Care Managers, referral agencies, families, advocates, and other stakeholders throughout the intake process.
Schedule intake appointments, program tours, assessments, trials, transportation, and admission meetings.
Verify Medicaid eligibility using the ePACES system.
Submit and process OPWDD admission documentation through the OPWDD CHOICES portal.
Maintain and update referral, admission, and discharge information within the Netsmart Electronic Health Record (EHR).
Prepare complete admission packets for Program Directors and Quality Management.
Coordinate and facilitate Intake Committee meetings, including scheduling, meeting materials, and meeting minutes.
Maintain accurate referral, admission, discharge, and census reports.
Monitor admissions to ensure compliance with OPWDD regulations and agency policies.
Build and maintain positive relationships with referral sources, Care Management organizations, hospitals, and community partners.
Represent Gateway Counseling Center at outreach events, provider fairs, and community meetings to promote agency services.
Collaborate with Program Directors, Quality Management, Nursing, Transportation, and other departments to ensure successful admissions.
Perform additional duties as assigned.

Minimum Qualifications

Associate's degree required; Bachelor's degree preferred in Human Services, Healthcare Administration, Social Work, Psychology, or a related field.
Minimum two (2) years of experience in admissions, intake, case management, healthcare administration, or OPWDD services.
Excellent organizational and time management skills.
Strong written and verbal communication skills.
Ability to manage multiple priorities while maintaining accuracy and attention to detail.
Proficiency with Microsoft Office (Word, Excel, Outlook).

Preferred Qualifications

Candidates with the following experience are strongly encouraged to apply:

Experience working with OPWDD programs and regulations.
Working knowledge of the OPWDD CHOICES system.
Experience submitting documentation through CHOICES.
Experience verifying Medicaid eligibility through ePACES.
Experience using Netsmart Electronic Health Record (EHR) or a similar EHR platform.
Experience coordinating admissions for Day Habilitation, Day Treatment, Residential, or other OPWDD services.
Knowledge of Medicaid eligibility requirements and OPWDD enrollment procedures.
Bilingual English/Spanish preferred.

Pay: $27.00 - $30.00 per hour
