Full Job Description

Under the general supervision of the Director of Navigation & Customer Services, coordinate activities and resources for the abortion access program including all administrative tasks and sites involved in the project. Coordinate meetings for patients with community health professional, including travel arrangements and expense reports. More so, the Patient Navigator will provide in-person assistance to the abortion patient’s needs. The patient navigator helps guide the patient through the abortion care system and works to overcome obstacles faced by the patient receiving the care and services they require, while working to improve the systems and processes that shape access at the clinical and ecosystem levels. An effective patient navigator should have a demonstrated ability to quickly and effectively synthesize patient needs and liaise with providers and local resources to create a unique patient care plan.

Position Overview: The Patient Navigator is a community health professional who provides field-based case management services to patients accessing Abortion care. This person is the main point of contact for abortion care services. The Patient Navigator builds strong relationships with patients in order to help them stay engaged in medical care and adhere to their guidance. The person in this position is committed to removing the patient’s barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting patient’s health. They work closely with the Abortion Access team, which may include clinician, nurses, and other clinical staff to support positive patient health outcomes.

ESSENTIAL FUNCTIONS Some of the responsibilities of the patient navigator may include:

1. Organize schedules and manage appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences.

2. Facilitate communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.

3. Inform the patient on financial aspects of care and link the patient to financial resources as needed.

4. Works with the patient to identify logistical needs to access care and link the patient to appropriate resources as needed.

5. Work in collaboration with: Other Patient Navigators, health care providers, center managers, other clinical support staff.

6. Accommodates interpretive language services when needed.

7. Facilitates pre- and post-abortion visits and ensures continuity of care and linkage to community resources.

8. Collects, tracks, and reports required program data to appropriate stakeholders.

9. Maintains strict confidentiality in accordance with agency policies.

10.Ensures adherence with PPMNJ’s "Privacy Guidelines," HIPAA compliance program, and PPFA’s MG&Gs, where applicable
