Easterseals of Northeast Ohio is seeking an energetic and compassionate part-time Activity Coordinator to work in our Adult Day Care to help to make a difference in our clients' lives. No weekends or holidays are required
POSITION SUMMARY:
The Activity Coordinator is responsible for planning, implementing, and evaluating a comprehensive, person-centered activity program that promotes the physical, cognitive, social, emotional, and recreational well-being of clients in the Adult Day Care program.
PRIMARY RESPONSIBILITIES:
- Develop, implement, and evaluate a monthly person-centered activity calendar that addresses clients' physical, cognitive, emotional, social, recreational, cultural, and spiritual interests and needs.
- Complete activity assessments for each client upon admission and update them as required to ensure activities reflect individual preferences, strengths, abilities, and goals.
- Develop individualized activity plans in collaboration with clients, families, and the interdisciplinary team.
- Facilitate individual and group activities that promote independence, wellness, socialization, community inclusion, and quality of life.
- Adapt activities to accommodate clients with varying physical, cognitive, sensory, and behavioral abilities.
- Encourage client choice and active involvement in planning and participating in activities.
- Coordinate therapeutic recreation, exercise, educational programs, arts and crafts, music, games, holiday celebrations, special events, and community outings.
- Coordinate volunteer participation and guest presenters to enhance programming.
- Plan, organize, and supervise community outings while ensuring client safety and compliance with agency policies.
- Collaborate with nursing staff, personal care staff, transportation personnel, and other team members to ensure activities support each client’s overall plan of care.
- Monitor client engagement and response to activities and modify programming as needed to maximize participation and therapeutic benefit.
- Maintain activity supplies, equipment, and storage areas in clean, safe, organized, and well-stocked conditions.
- Assist with decorating the center to create a welcoming, engaging, and seasonal environment.
- Ensure activity areas comply with all infection prevention, safety, emergency preparedness, and environmental standards.
- Assist with mealtimes and provide direct client care such as walking, transferring, and toileting as needed to support efficient daily operations.
- Maintain updated knowledge of regulatory standards as they relate to Ohio Medicaid, Ohio Department of Aging, and Commission on Accreditation of Rehabilitation Facilities and other regulatory bodies as they apply to all home and community-based services.
- Adhere to the standards of conduct, established Code of Ethics, confidentiality, privacy, and security of protected health information, and all policies and procedures of Easterseals.
- Attend mandatory staff meetings, in-service training, and competency sessions as required for the position.
- Perform other related duties as assigned.
QUALIFICATIONS:

- Minimum of 1 year of experience working in Adult Day Care.
- Must have a valid driver’s license and reliable transportation.
- Must have proof of auto insurance.
- A criminal background check must meet the requirements of the State of Ohio
- Bachelor’s degree in social work or social services preferred.

- Strong observation skills to recognize changes in the clients' physical, emotional, or cognitive status and report concerns promptly.
- Creative problem-solving skills and the ability to adapt activities to meet individual client needs.
- Strong organizational and time management skills with the ability to manage multiple priorities and meet deadlines.
- Ability to exercise sound judgment and maintain confidentiality in accordance with HIPAA requirements.
- Displays a high level of motivation and a self-imposed standard of excellence.
EQUIPMENT USED/MACHINERY REQUIREMENTS:
- Proficient use of standard office equipment, Microsoft Office applications, applicable software programs, and other technology required to perform assigned duties.
PHYSICAL REQUIREMENTS:
- Ability to lift, carry, push, or pull up to 50 pounds to assist in the delivery of client care.
- Frequently requires sitting, standing, walking, bending, squatting, kneeling, reaching, and a full range of body motion.
- Potential exposure to blood and body fluids, and communicable diseases.
FLSA Status: Non-Exempt/ $17 per hour
Classification: Part-Time, 20-29 hours per week
