Position: Per Diem Certified Nursing Assistant (CNA) / Home Health Aide (HHA)
Employment Type: Per Diem
Location: Orange County and surrounding areas

About IntelliCare Home Health Agency

IntelliCare Home Health Agency is an ACHC-accredited and Medicare-certified home health agency dedicated to providing compassionate, patient-centered care in the comfort of our patients' homes. We are seeking caring, dependable, and motivated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to join our team on a per diem basis.

Position Summary

The Home Health CNA/HHA provides personal care and supportive services to patients in their homes under the direction and supervision of a Registered Nurse (RN) or licensed therapist, in accordance with the patient's individualized Plan of Care and agency policies. The CNA/HHA plays a vital role in helping patients maintain their independence, dignity, comfort, and safety.

Responsibilities

Assist patients with activities of daily living (ADLs), including:
Bathing and personal hygiene
Dressing and grooming
Toileting and incontinence care
Oral care
Skin care
Assist with safe transfers, positioning, ambulation, and range-of-motion exercises.
Assist with light meal preparation and feeding when appropriate.
Take and record vital signs, weight, and other observations as directed.
Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse.
Reinforce patient safety measures, including fall prevention and infection control practices.
Maintain a clean, safe, and organized patient environment.
Document care provided accurately and submit required documentation promptly.
Maintain patient confidentiality and comply with HIPAA regulations.
Communicate professionally with patients, families, and members of the interdisciplinary care team.

Qualifications

Current California Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certification.
Current CPR/BLS certification.
At least one (1) year of patient care experience preferred; home health experience is a plus.
Valid California driver's license, reliable transportation, and current automobile insurance.
Ability to travel throughout the assigned service area.
Compassionate, dependable, and professional demeanor.
Excellent communication and interpersonal skills.
Ability to work independently while following the established Plan of Care.

Physical Requirements

Ability to lift, transfer, and assist patients safely.
Ability to stand, walk, bend, kneel, and reach for extended periods.
Ability to lift up to 50 pounds with appropriate assistance.
Ability to perform duties in a variety of home environments.
