What You'll Do

As a Case Manager, you'll help residents living in nursing homes stay as healthy as possible.

You'll work closely with our Nurse Practitioners, facility staff, and families to make sure every patient gets the care they need.

Your day may include:

Visiting members in skilled nursing facilities
Talking with nurses, providers, and family members
Helping coordinate medical care and services
Identifying changes in a member's condition early
Working to prevent unnecessary hospital visits
Documenting your visits and care plans
Building strong relationships with facility staff

Every day is different, and every day you have the chance to improve someone's life.

What We're Looking For

Current Ohio RN or LPN license
Experience in long-term care, case management, or managed care is helpful
Good communication skills
Strong organization and time management
Ability to work independently
Compassion for older adults
Active and unrestricted drivers license
