Registered Nurse (IMT) – Behavioral Health | $107K

Mobile Community-Based Care Across NYC! Intensive Mobile Treatment Role!

Bronx, Manhattan, Brooklyn & Queens Coverage

Are you a Registered Nurse looking to do meaningful, community-based work in behavioral health? We are seeking a Psych & Behavior Health RN to join a dynamic, trauma-informed, multidisciplinary team that serves individuals experiencing street homelessness, serious mental illness, and substance use disorders. This is an Intensive Mobile Treatment Role(IMT) Role!

This is not a typical healthcare role. You’ll be practicing in non-traditional settings—on the streets, in shelters, community spaces, and homes—wherever participants are. The right candidate is someone who is deeply mission-driven, thrives in the field, embraces harm reduction, and wants to make a lasting difference in people’s lives without trying to “fix” them.

What You’ll Do:

Community-Based Nursing & Harm Reduction Care

Conduct psychiatric and physical health assessments in field settings, including streets and shelters
Provide nursing oversight for a caseload of 27 participants with complex behavioral health needs
Administer medications (IMs, vaccines, phlebotomy, PPDs) and monitor for side effects
Teach wellness strategies and educate clients—not just treat symptoms
Collaborate with the team’s psychiatrist and nurse practitioner to manage care holistically
Use systems like PSYCKES, Healthix, and MAPP to support planning and coordination
Deliver crisis intervention services on a phone-based rotating on-call schedule (no field response required)
Work across all NYC boroughs, primarily Bronx and Manhattan, using public transportation with a company-paid transit card

Care Coordination & Participant Advocacy

Develop individualized care plans in alignment with participant self-determination and recovery goals
Make referrals, coordinate appointments, and advocate for clients within various systems (healthcare, housing, legal, etc.)
Serve as a trusted resource to the IMT team on medical and wellness issues
Maintain accurate, timely documentation using AWARDS

Schedule & Location:

Hours: Monday – Friday, 9:00 AM – 5:00 PM
Flexible to accommodate participant needs (some mornings/evenings/weekends)
In-person clinical meeting on Tuesdays at the IMT Bronx office (Bronx NY 10451). 9AM
On Call: Rotating phone-only coverage with two other team members
Setting: Hybrid Field/Home-based
Transportation: Must be comfortable navigating NYC via public transit—

no car needed

Qualifications:

Education & Licensure

Registered Professional Nurse (RN) licensed in New York State
Eligible for Medicare/Medicaid participation
Must pass NY State Justice Center background/fingerprint clearance

Experience

Minimum 2 years RN experience in behavioral health settings
Experience working with homeless individuals and those with substance use and serious mental illness
Comfortable working independently in unpredictable, non-clinical settings

Skills & Values

Strong documentation, communication, and advocacy skills
Commitment to harm reduction and trauma-informed care
Comfort with mobile/field-based work
Bilingual Spanish-speaking preferred
