Job Title: Palliative Care Manager (LMSW) – Full-Time
Location: Grand Rapids, MI
Schedule: Full-Time | Monday-Friday | 8:30 AM - 5:00 PM (Manager On-Call Rotation Required)
Salary: $75,000 - $85,000 per year
Mandatory Qualifications
- Master's Degree in Social Work (MSW) required
- Active Licensed Master Social Worker (LMSW) or eligible for Michigan licensure upon acceptance
- Minimum 3 years of Clinical Social Work experience
- Experience in Hospice, Palliative Care, Home Health, Case Management, or Behavioral Health preferred
- Previous leadership, supervisory, or program development experience preferred
- Strong knowledge of Medicare, Medicaid, regulatory compliance, accreditation standards, and payer requirements
- Experience using Electronic Medical Records (EMR/EHR) systems
- Proficiency with Microsoft Office Suite
Roles and Responsibilities
- Lead the startup and development of a new palliative care program
- Design and implement operational workflows, policies, and clinical processes
- Manage program budget, financial performance, and operational efficiency
- Supervise, mentor, and support interdisciplinary palliative care staff
- Collaborate with the Medical Director and executive leadership
- Ensure compliance with Medicare, Medicaid, commercial insurance, and accreditation requirements
- Oversee provider credentialing and payer contracting activities
- Coordinate patient referrals, scheduling, documentation, and care transitions
- Deliver clinical social work services during program startup and as needed
- Develop quality improvement initiatives and performance metrics
- Build relationships with referral sources and community healthcare partners
