Epic Heart & Vascular Care is seeking an exceptional Nephrology Nurse Practitioner to join our expanding Cardio-Kidney-Metabolic (CKM) Center of Excellence to work in close professional supervision of nephrologist and cardiologist.

Develop individualized treatment plans.
Educate patients regarding kidney health, cardiovascular risk reduction, diabetes management, nutrition, and lifestyle modifications.
Order and interpret laboratory studies, imaging, and diagnostic testing.
Promote preventive medicine and long-term disease management.
Build meaningful relationships with patients and families.
