Location: Southwest Minnesota
Compensation: $110,000-$210,000+ annually
Bonus: $15,000 Sign-On Bonus

You'll have the opportunity to serve patients throughout Southwest Minnesota while enjoying a level of autonomy that traditional settings simply can't offer. What You'll Be DoingDeliver Exceptional Rehabilitation Services

Perform comprehensive patient evaluations and develop individualized care plans
Deliver evidence-based therapeutic interventions
Provide caregiver education and equipment recommendations
Collaborate with nurses, physicians, and interdisciplinary teams
Accurately document visits and monitor patient progress

Required Qualifications

Active Minnesota Physical Therapist license
Willingness to travel throughout Southwest Minnesota
Strong communication and clinical reasoning skills
Ability to work independently and manage your schedule effectively

Preferred Qualifications

Home health experience
Experience with medically complex patients
Passion for relationship-based, patient-centered care
