JOB TITLE: STAFF NURSE
DEPARTMENT: NURSING
REPORTS TO: UNIT MANAGER, NURSING SUPERVISOR, NURSING ADMINISTRATION
SUMMARY DESCRIPTION:
A Staff Nurse is a Registered Nurse (RN) or Licensed Practical Nurse (LPN) whose primary responsibility is to delivers safe and therapeutic nursing care using the Nursing Process and in accordance with the Nurse Practice Act, DOH Regulations, State Regulations and the policies and procedures of Fair Acres.
PRIMARY RESPONSIBILITIES:
- Is confident and flexible in ability to practice nursing on any unit throughout the facility, and maintains a clean, safe, and orderly environment.
- Maintains open, professional communication with the Unit Manager, Nursing Supervisor, residents, visitors and employees.
- Supports the goals and objectives of the Nursing Department and Administration of Fair Acres, and participates on committees as requested.
- Ensures effective organization of assignments by establishing priorities and setting goals.
- Evaluates and reports all significant symptoms, reactions, and changes in a resident's condition.
- Records pertinent information and assures the continuity of accurate and timely documentation to facilitate communication among services.
- Performs nursing functions, such as:
- Administration of medications and treatments
- Activities of daily living
- Rehabilitative techniques to restore and/or maintain resident independence
- Maintains standard precautions.
- Provides end of life care.
- Participates and monitors the TAPS program.
- Collaborates with members of the interdisciplinary health care team in providing quality care, and documentation of resident goals.
- Promotes and models a cooperative team effort among staff and peers.
- Maintains and ensures resident and staff confidentiality to be compliant HIPAA.
- Participates in Staff Development programs as mandated by Fair Acres and seeks additional self-enrichment and professional growth through continuing education.
- Ensures correct use of equipment and supplies, and notifies appropriate departments when maintenance is required.
- Assumes other duties as deemed necessary by the Unit Manager and/or Nursing Supervisor.
QUALIFICATIONS:
- Graduate from an accredited nursing program
- Current Pennsylvania Nursing License
- Current BLS CPR (In-person)
PHYSICAL REQUIREMENTS:
- Must be able to lift/transfer residents
- Must be able to move, unencumbered, about the facility
Fair Acres is regulated by State and Federal law. It has an active Compliance Program to enable Fair Acres to meet these State and Federal requirements. You will be trained in compliance components that relate to the performance of your job. Failure to adhere to the compliance components of your job may result in discipline, up to and including dismissal.
STAFF NURSE RESPONSIBILITIES
Each nurse will have the responsibility to complete all aspects of care related to the residents. The following information is to be used as a guideline for this purpose and is not all inclusive. Always follow the current practices and facility policy.
- Count narcotics with off-going shift.
- Check emails each shift.
- Check that all necessary equipment is available and functioning properly (all emergency equipment).
- When on a wander guard unit, use transmitter to check bracelets are working and all doors lock.
- Obtain report from off-going shift and make walking rounds. Check resident's condition/overall condition of unit. * Clear outstanding items in EPS (Electronic Pharmacy System).
- Print BM report, follow bowel protocol.
- Ensure staff is on unit as scheduled, direct nursing assistants to begin arrival care. Assists with rehabilitative nursing techniques as needed.
- Complete dally assignment sheet, schedule breaks and mealtimes, assign residents to be fed at meals, dayroom duty, weights, resident smoking schedules (as care planned), baths and showers, vital signs, scheduled appointments, etc. Give verbal report to staff.
- Check appointment calendar. Ensure transfer paperwork is complete and contacts have been notified. Ensure all recommendations are reviewed with physician and all follow-up is completed.
- Check in-service calendar; assign staff to attend in-services.
- Check lab book:
- If resident had PT/INR drawn, note on 24-hour report.
- Notify lab to collect all specimens from refrigerators.
- Check labs/tests scheduled
- Include this information on the 24-hour report and in report given to nursing assistants. Initiate 24-hour report.
- Physician rounds.
- Check doctor's rounds sheet for RPO's and doctor's book for issues to be addressed.
- Print out the EHR (Electronic Health Record) report to determine all new and discontinued orders.
- Review and update nursing care plans and Fair Facts as necessary.
- Ensure completion and documentation of vital signs.
- Notify contacts of any changes in plan of care.
- Administer medications and perform treatments as ordered. Maintain date compliance with all medications and treatments.
ROUTINE DOCUMENTATION
- A nurse's note is required for all residents noted on 24-hour report, skilled for Medicare A and as needed.
- Complete all new admission and post hospital documentation.
- Use admission and post hospital checklist.
- Complete IDC updates.
- Complete body checks and appropriate follow-up and notifications when necessary.
- Measure and document wounds weekly. If new skin areas are identified, follow wound protocol to document and initiate treatment.
- Assist nursing assistants with care, including toileting, transferring, and answering call lights when staff are occupied with other residents.
- Review trays and assistive devices with diet list prior to meal pass.
- Update seating chart for new admissions and as needed.
- Make walking rounds at least twice per shift. Observe for proper turning and positioning of residents.
- Participate in TAPS.
- Ensure resident has LOA physician's order prior to leaving facility. Complete release of responsibility forms for any resident going out on pass. Document accordingly.
- Check kiosk for nursing assistant documentation compliance throughout shift.
- Nursing assistants are to report off at the end of the shift. Call nursing assistants to desk to obtain updated report on their residents.
- Complete all documentation as soon as possible.
- Inform Unit Manager/Supervisor immediately of any incidents, change in resident's condition or problems with staff.
- Listen and answer any family or resident concerns. Document and refer concerns to the Unit Manager/Supervisor.
- Ensure all PRN medications administered have been documented as effective or not effective with appropriate follow-up.
- Restock medication and treatment carts,
- Chart equipment/report faulty equipment.
- Staff to remain on unit until replacements arrive or until released.
- Monitor new employees through unit orientation process.
- In the event the EHR is not functional, all documentation must be completed on appropriate Fair Acres forms.
7-3 Nurse:
- Complete all IDC assessments prior to ARD date.
3-11 Nurse:
- Maintain and update PT/INR record daily.
- Turn off hall lights at 10pm.
- Complete all care plan updates prior to IDC meeting,
- Significant changes and annuals require recopying of all care plans. **Quarterlies require revision and updates as necessary..
11-7 Nurse:
- Create physician billing sheet.
- Complete 24-hour chart check.
- Compile vital sign reports for RPO's as needed.
- Update all orders for RPO's.
- Change tube feeding sets and syringes on all residents requiring tube feeding. Date all supplies.
- Change all respiratory equipment and supplies weekly per schedule. Date all supplies.
- Fill out census in black ink.
- Check pantry refrigerator and dispose of outdated items.
- Thoroughly clean pantry refrigerator every Saturday night.
- Check medication room refrigerator and dispose of expired medications.
- Thoroughly clean medication refrigerator every Saturday night.
