We are seeking a dedicated and compassionate Speech Therapist to join our team. The ideal candidate will have a strong background in speech therapy, particularly with the geriatric population . The Speech Therapist will work closely with patients, their families and the rest of the Home Health team to develop personalized treatment plans that promote effective communication and enhance overall quality of life.

Join our team as a Casual Speech Therapist where you can make a meaningful impact on the lives of individuals facing communication challenges!

Responsibilities

1. Providing excellent customer service to the patients and to coworkers by demonstrating respect and concern.
Performs comprehensive evaluations of patients with communication and/or swallowing disorders.
Develops the plan of care cooperatively with the physician, patient, and family with revisions made based on patient condition.
Provides therapeutic intervention to improve communication/cognitive and/or swallowing disorders.
Plans with the patient, family, and other team members for the patient’s discharge from service.
Completes all documentation on a timely basis which includes but is not limited to the initial screening/evaluation forms, progress notes, care plans, and discharge notes.
Prepares patient’s record for recertification if necessary.
Reports to case manager a summary of the patient’s condition and any other pertinent information.
Works in conjunction with the office manager to schedule all appointments as needed.
Adheres to all policies set forth in the Speech Pathology Practice Acts/American Speech-Language Hearing Association to maintain safe clinical practice.
Maintains a professional standard of conduct, projecting a positive image of the agency at all times.
Complies with infection control and safety policies and procedures.
Adheres to all policies and practices of the agency to maintain established standards of care.
Performs additional duties as assigned.

Skills and Abilities

1. May be required to transport a patient from one setting to another

2. Excellent interpersonal skills

3. Knowledge of basic evaluation and treatment documentation skills

4. Ability to input billing information into computer

5. Lifting: Regularly lifts and moves patients, equipment, and patient supplies. Frequently lifts up to 40 pounds. Occasionally lifts and moves patients

6. Carrying: Carries medical supplies, and other items, weighing up to twenty pounds.

7. Pushing/Pulling: Pushing and pulling is required when administering patient therapy and care.

8. Climbing: Ability to use stairs, curbs, and walkways in order to access patient home.

9. Bending: Required when administering patient care.

10. Reaching/Turning/Twisting: Required when administering care.

11. Repetitive Motions: Hands-required for certain aspects of patient care: simple grasping, fine manipulation. Able to walk and stand for various duties.
