We are looking for a motivated, compassionate, and highly organized Health / Nutrition Coach to join our team. This role is ideal for someone who loves helping patients turn medical recommendations into real-life habits, stay accountable, and feel supported throughout their health journey.
Job Summary / Position Overview
The Health / Nutrition Coach works directly with patients to help them improve nutrition, lifestyle habits, body composition, metabolic health, energy, performance, and long-term wellness outcomes.
This person will partner closely with our providers to help patients understand their care plan, stay engaged between visits, and take consistent action toward their goals.
This is not a passive education role. We are looking for someone who can coach, motivate, follow up, problem-solve, and help patients actually implement the plan.

- Is comfortable working closely with providers, medical assistants, and the rest of the care team

- Can coordinate prescriptions and communicate professionally with pharmacies

- Is not interested in functional medicine, nutrition, or lifestyle-based care
- Believes coaching is just telling people what to do instead of helping them change behavior

- Does not want to grow, learn, or improve as the clinic grows
Key Responsibilities
Patient Coaching
- Conduct health and nutrition coaching sessions with patients
- Help patients understand and implement provider recommendations
- Create practical nutrition, lifestyle, and habit-based action steps
- Support patients with weight loss, body composition, energy, blood sugar balance, inflammation, gut health, performance, and long-term wellness goals
- Help patients identify barriers, patterns, and behaviors that are keeping them stuck
- Provide accountability, encouragement, and clear next steps between provider visits
- Help patients build consistency with food, hydration, sleep, movement, stress management, and routines
Nutrition and Lifestyle Support
- Review patient nutrition patterns and help them make realistic improvements
- Educate patients on foundational nutrition principles within your scope
- Support patients through structured nutrition programs
- Help patients understand how food choices connect to energy, weight, digestion, inflammation, and metabolic health
- Use tools such as food logs, habit trackers, body composition data, and patient goals to guide coaching conversations
- Help patients move away from all-or-nothing thinking and toward sustainable behavior change
Patient Experience Ownership
- Make sure every patient leaves coaching sessions knowing exactly what to focus on next
- Help patients feel heard, understood, and supported
- Reduce confusion by explaining next steps clearly and simply
- Anticipate common barriers and help patients problem-solve before they fall off track
- Follow up with patients in a way that builds trust, consistency, and accountability
- Help patients feel like Med Matrix is the clinic that is truly walking with them through their health journey
Team Collaboration
- Communicate patient progress, concerns, and barriers to providers and provider assistants
- Document coaching sessions clearly and professionally in the EMR
- Participate in team huddles, trainings, and process improvement
- Support provider recommendations by helping patients implement the plan between visits
- Collaborate with the team to improve patient retention, program completion, and overall outcomes
- Help identify patients who need additional support, follow-up, or escalation to a provider
Growth and Continuous Improvement
Look for ways to improve coaching systems, patient education, and follow-up workflows
Help build a consistent nutrition coaching experience across the clinic
Participate in training related to functional medicine, metabolic health, behavior change, and patient communication
Support the growth of nutrition programs, memberships, and patient education resources
Bring ideas, ownership, and energy to a team that is always improving
Required Skills and Qualifications
- Experience in health coaching, nutrition coaching, wellness coaching, fitness coaching, personal training, or a related field required
- Nutrition, health coaching, personal training, or wellness certification preferred
- Strong communication skills and a patient-centered mindset
- Excellent organization, documentation, and follow-through
- Comfortable discussing nutrition, lifestyle, habits, and behavior change
- Ability to motivate and hold patients accountable in a supportive way
- Comfortable working in a fast-paced clinical environment
- Ability to work as part of a team and follow established clinic systems
- Interest in functional medicine, regenerative medicine, metabolic health, and long-term wellness
- EMR experience preferred but not required
