Recruitment Bonus Incentive: $2,500

POSITION SUMMARY:

Seeking an Empowerment Ally to work with LGBTQ+ patients at the Transgender Center of the Rockies, Denver Element (LGBQ behavioral health program), and Bee the Vibe (harm reduction program). The Empowerment Ally will provide specialized support focusing on harm reduction and education around substance use and misuse and other co-occurring disorders. This role involves advocating for the health and wellness of LGBTQ+ individuals, offering personalized guidance, and collaborating with healthcare providers to ensure inclusive and comprehensive care. The Empowerment Ally builds trusting relationships, connects patients with community resources, and conducts educational outreach to promote health and wellness.

ESSENTIAL JOB FUNCTIONS:

· Collaborate with the client to monitor individualized treatment plan progress.

· Provide extensive case management services to patients as needed or assigned to support whole person health care.

· Coordinate intakes for interested patients as well as provide clinical support during open office hours.

· Maintain accurate and timely clinical records consistent with Medicaid, private insurance and the Behavioral Health Administrative Service organizations (BHASO)’s standards in all areas including, but not limited to: admission summaries, assessments, daily charting, weekly clinical staffing notes, and discharge summaries within the agency electronic health record system (EHR). Adhering to mandatory deadlines.

· Required to maintain weekly relationships of the patients assigned to you and maintain an active caseload ensuring billable behavioral health services.

· Create and implement outreach plans about harm reduction to reduce opioid overdoses and spread of infectious diseases (HIV/HCV/STI) and provide HIV/STI rapid testing.

· Recruit target populations into the Element, Bee the Vibe/Harm Reduction and Transgender Center of the Rockies, the Hive and other MHBHC programs by providing testing, case management and harm reduction groups/coaching.

· Coordinate and lead large events held at MHBHC including: Overdose Awareness Month, Overdose Awareness Day, World AIDS Day, Pride (Denver and Summit County), Transgender Day of Visibility/Remembrance and other events as they arise/are created.

· Oversee Marsha’s Closet, Food pantry and provide linkages to MHBHC programming and interventions for community members.

· Participate in team/department meetings, in-services to learn about other services such as Mind Lift (Ketamine assisted psychotherapy), and supervisory sessions as required.

· Collaborate with all Transgender Center of the Rockies, Element and Bee the Vibe/Harm Reduction staff to ensure interdisciplinary care.

· Use street outreach strategies to provide harm reduction and intervention to potential and current patients by meeting them in public spaces such as churches, bars, parks, medical facilities and van outreach including fairs etc

· Ability to provide and support with Peer Services.

· Professionally represent the agency and program at community resource fairs.

· Treat all patients with respect and equality.

· Perform other duties as assigned.

QUALIFICATIONS:

· Familiarity with LGBTQ+ cultural and health disparities including those living with HIV.

· Experience in sexual health work and/or Peer Recovery services.

· Knowledge of community resources.

EDUCATION AND EXPERIENCE:

· Education: Preferred Bachelor’s degree in human services with substance use licensure in CAT, CAS

· Experience working with substance use patients highly desired. Experience working with marginalized and underserved communities, particularly transgender and gender expansive, LGBQ+ behavioral health communities.

OTHER REQUIREMENTS:

· Valid Driver’s License and car insurance by date of hire

· Ability to undergo background check due to caring for children, adolescents and families.

· Some travel around the Denver Metro Area

· Some evening and weekend work for events

· This is a full-time in office position (40 hours a week).

This is a full-time, salaried position with benefits. Salary is commensurate with experience.
