Full Job Description

OVERVIEW

We are seeking a highly motivated and clinically skilled Nurse Practitioner (NP) or Physician Assistant (PA) to serve as an Advanced Practice Provider (APP) and Clinical Coordinator supporting specialty programs across a growing multi-state network.

This hybrid role is ideal for an experienced provider who thrives in a collaborative, fast-paced environment and is passionate about delivering high-quality care for patients with complex surgical conditions. The APP will work closely with physicians, clinical teams, and operational leadership to support patient evaluation, longitudinal patient management, care coordination, clinical standardization, and program development across multiple specialty service lines and markets.

The position is primarily telehealth and remote-based, with flexibility to travel as needed to support in-person specialty clinics, physician training initiatives, program activations, and multi-site clinical operations. The position is expected to be on-site for clinics as determined by physician and program needs.

The ideal candidate demonstrates strong clinical judgment, exceptional organizational and communication skills, adaptability, and a commitment to delivering an outstanding patient experience within a growing national specialty care model. This provider will serve as a key clinical leader within specialty programs, managing complex patient populations, supporting surgical decision-making, and ensuring continuity of care throughout the patient journey.

KEY RESPONSIBILITIES

Clinical Care Delivery

Conduct comprehensive patient evaluations through both telehealth and in-person visits across specialty programs Independently manage patients longitudinally throughout the continuum of care, from initial evaluation through post-operative recovery Develop, implement, and adjust individualized treatment plans in collaboration with physicians

and multidisciplinary teams Collaborate closely with surgeons and physician teams to evaluate complex surgical conditions and determine appropriate treatment pathways Participate in surgical candidacy assessments and clinical decision-making discussions for complex specialty patients Identify appropriate surgical candidates and coordinate escalation, workup, and physician evaluation as appropriate Manage perioperative and postoperative care, including clinical monitoring, symptom management, recovery planning, and follow-up care Order, review, and interpret diagnostic studies including imaging, laboratory testing, and specialty evaluations Provide advanced clinical recommendations and evidence-based treatment guidance to physicians, patients, and care teams Provide thorough patient education regarding diagnoses, treatment pathways, surgical expectations, and recovery planning Deliver consistent, compassionate, and high-quality patient-centered care across all clinical interactions

Care Coordination & Patient Navigation

Serve as the central clinical coordinator for complex patient cases across multiple providers, specialties, facilities, and states Serve as a primary clinical liaison guiding patients throughout the full continuum of care Coordinate consultations, diagnostics, clearances, procedures, follow-up appointments, and multidisciplinary care needs Partner closely with Specialty Program Central Intake, surgical coordinators, office administrators, and physician teams to ensure seamless patient flow and communication Ensure patients are clinically and administratively prepared for evaluations, procedures, and surgical interventions Identify and proactively address barriers to care including scheduling, insurance requirements, travel logistics, and coordination across multiple sites Collaborate with research and clinical teams to support accurate and compliant clinical data collection when applicable Maintain continuity of care across multiple providers, facilities, and states

Clinical Leadership & Specialty Program Development

Support physicians and operational leaders in the implementation and expansion of specialty programs across multiple markets Assist in standardizing patient evaluation processes, clinical workflows, treatment pathways, and care coordination models across sites Participate in onboarding and integration of new specialty program initiatives and operational workflows
Support clinical readiness, operational coordination, and patient flow during program activations and expansion efforts Contribute to the development of clinical protocols, best practices, patient education resources, and specialty program standards Provide clinical insight and recommendations to support program growth, quality improvement, and patient outcomes

Roadshows & Multi-Site Clinical Support

Participate in specialty clinics and physician roadshows in developing and expanding markets Provide in-market clinical support during early-stage program implementation and growth initiatives Assist with patient evaluations, care coordination, and clinical workflow execution during specialty clinic activations Maintain flexibility to support multiple locations based on operational priorities and patient needs

Operations, Compliance & Documentation

Maintain timely, accurate, and compliant documentation within EMR systems Ensure adherence to HIPAA regulations, telehealth standards, and state-specific clinical requirements Support centralized intake workflows, patient tracking systems, and quality improvement initiatives Participate in process improvement efforts focused on enhancing patient outcomes, operational efficiency, and clinical excellence Assist with tracking patient outcomes, quality metrics, and specialty program performance indicators

QUALIFICATIONS

Active Nurse Practitioner (NP) or Physician Assistant (PA) license required Willingness to obtain and maintain multi-state licensure Minimum of 4 years of experience in surgical, procedural, or specialty care settings preferred Strong clinical judgment and experience independently managing medically complex patient populations Experience developing treatment plans, managing perioperative and postoperative patients, and coordinating multidisciplinary care preferred Experience in telehealth, patient navigation, care coordination, and multi-site clinical operations preferred Demonstrated ability to manage complex patient cases with a high degree of autonomy while collaborating effectively with physicians and multidisciplinary teams
Excellent communication, organizational, and interpersonal skills Willingness and flexibility to travel as needed to support specialty clinics and program operations across multiple locations

Compensation & Benefits:

Pay Range: $140,000-$170,000 annually

the starting rate within this range for this role varies depending on a number of factors, including a candidate’s qualifications, skills, competencies, experience, and location.
