Full Job Description

Position Summary:

The Case Manager will provide trauma-informed, culturally responsive case management

services to immigrant individuals and families for all case management needs. This role includes assisting clients in navigating a variety of systems such as healthcare, housing, education, employment, and other needed services.

The ideal candidate is bilingual (Spanish/English), experience working with vulnerable

communities, and be skilled in navigating resources and building trusting relationships. This

position requires a strong understanding of trauma-informed practices, discretion, and a

commitment to social justice.

Key Responsibilities

Conduct comprehensive intakes and needs assessments to understand the client’s needs and develop service plans tailored to develop and implement individualized service plans.
Provide trauma-informed, client-centered case management including safety planning,
housing support, public benefits assistance, healthcare navigation, legal referrals, and crisis intervention.
Act as a liaison and advocate for clients with public agencies, legal providers, healthcare systems, educational institutions, and community partners.
Assist clients with budgeting and basic financial literacy; connect them to emergency assistance and ongoing support resources.
Maintain accurate and confidential records in digital databases; complete reports required by program or grant funders.
Coordinate and participate in interdisciplinary meetings, case reviews, or community task forces to improve client outcomes.
Provide follow-up support to ensure continued progress and update service plans as Needed.
Ability to de-escalate mental health crisis situations and identify resources to mitigate future crises.
Participate in outreach efforts, including public presentations and resource fairs, to educate the community about trafficking and available services.
Facilitate or co-lead life-skills or resource education workshops for clients when applicable.
Occasionally travel with or on behalf of clients to appointments and external meetings.
Attend staff meetings, supervision, and professional development training.

Qualifications

Fluent in English and Spanish (spoken and written) – required
Bachelor’s degree in Social Work, Psychology, Human Services, or a related field; equivalent work experience considered
Minimum of 1 years’ experience in case management, victim services, or related fields, preferably with immigrants.

Skills and Competencies:

Knowledge of trauma-informed care and human trafficking dynamics
Excellent communication, problem-solving, and organizational skills
High level of cultural humility, empathy, and professionalism
Ability to work independently while maintaining collaboration with a team
Valid driver’s license and reliable transportation required.
Ability to drive a 15 van passenger van

Working Conditions:.

Local travel to program sites and partner locations
Required to work outside of normal business hours, in order to meet the needs of the client
While performing the duties this job requires, the employee is regularly required to sit, to talk, to hear. The employee is occasionally required to stand, walk and reach with hands and arms. Employees may occasionally lift and/or move objects of up to 20 pounds.
