Full Job Description

Program Overview

The Health and Education Alternatives for Teens (HEAT) Program, established in 1992, is a multidisciplinary HIV care program at SUNY Downstate Health Sciences University serving adolescents and young adults ages 13 to 29. HEAT provides comprehensive medical care, mental health services, case management, and supportive services through an integrated, youth-centered model that promotes engagement in care, reduces barriers to services, and supports optimal health outcomes.

Client Navigator Position Description

Desired Starting Date: August 2026

Salary Range: $45,000 - $48,000 a year (.80 FTE; 4 days per week)

Job Location: On-Site. Mon, Wed, Thurs, Fri. SUNY Downstate Medical Center, 450 Clarkson Avenue, Brooklyn, NY 11203

Responsibilities:

Recruit, enroll, and orient eligible participants into the BEY Program.
Develop trusting relationships with participants through ongoing communication, engagement, and support.
Conduct participant outreach, retention, and re-engagement activities to promote continuous HIV care.
Assist participants in accessing HIV medical care, behavioral health services, and community resources.
Coordinate referrals and assist participants in addressing barriers to care, including transportation, benefits, housing, and other supportive services.
Provide appointment reminders, follow-up, and care coordination using approved communication platforms.
Support participant engagement in individualized counseling sessions and adherence activities.
Maintain accurate documentation of client encounters and services in electronic health record and reporting systems.
Participate in multidisciplinary team meetings, quality improvement activities, and required trainings.
Perform other duties as assigned.

Minimum Requirements:

High school diploma or equivalent with commensurate experience in client navigation, case management, outreach, peer support, behavioral health, or a related human services field.
At least one year of experience providing client navigation, care coordination, outreach, case management, or related direct services.
Demonstrated experience working with and providing affirming, culturally responsive services to adolescents and young adults, including people living with HIV and LGBTQ+ communities.
Strong interpersonal, organizational, and communication skills.
Ability to establish rapport with participants while maintaining professional boundaries.
Proficiency with Microsoft Office and electronic documentation systems.

Preferred:

Certification or training in mental health, peer services, community health work, substance use counseling, or a related discipline.
Knowledge of HIV care, community resources, and healthcare systems in New York City.
Experience with motivational interviewing, trauma-informed care, strengths-based service delivery, or harm reduction.
