The job that was described was nothing like my actual experience. Management is constantly changing protocols to meet numbers with blanket approaches rather that targeted strategies. Huge emphasis on numbers and a lack of compassion for overworked employees, especially the field staff. Office team members are generally expected to do the job of 2 people. Planned days off for PTO are taken away if management feels they can't cover your responsibilities. 

Upper management continually takes on more patients without the staff to support them. High pressure sales on sick patients, and pushing recertification for patients who don't need or want services anymore. Employees regularly get sick from the stress and high intensity work load, employees are pushed to always be meeting the next highest productivity numbers and training is lacking. Everyone is so stressed that asking management questions to clarify what wasn't covered in training will often be met with a "don't bother me" attitude. One of my supervisors literally told me "I usually just ignore you", not a great strategy for keeping valuable talent. Leadership is disorganized, mixed messages reign supreme, often leaving employees feeling confused and as though they're doing something wrong. Instead of analytical fact based approaches to solving real problems, you get platitudes on time management in long unproductive meetings without concrete strategies on how to meet goals. 

Ideas from lower level staff on how to solve problems is generally not taken under advisement and service failures are often blamed on those same overworked lower level staff. There's a lack of accountability and poor division of labor. There is no clear chain of command, high turnover and it's often difficult to figure out who is responsible for what. 

Many long time employees have serious health problems from the stress of the job, and lack of work/life balance, one employee I worked closely with had never been able to take a 2-week vacation during their entire over a decade tenure. Pay is lacking for the amount of work expected and benefits are great, but you generally aren't granted the time to use any of it. So on paper it looks nice, but in practice you aren't really able to use it so it's basically an empty promise. 

A culture of mistrust and micromanagement leaves morale low. Management will often impugn the character of employees to who need to take a sick day or just leave the company all together because of the toxic and (at times) even abusive work environment. Employees were encouraged to drink alcohol to manage the stress of the job, as though it was normal to have employees in tears from the pressure and unrealistic expectations. 

You generally need overtime to complete all of the tasks required for the day but management really frowns on overtime. If I could only use one word to describe my time here it would be: whiplash.

Positive feedback: Hearts are in the right place with majority of staff. A greater emphasis on clear directives is needed, with nuance and understanding everyone's role in patient care which is the ultimate goal. Everyone that I personally encountered were totally dedicated to doing their job well to (unrealistic) company expectations. Though I personally was treated quite poorly by a number of employees, not once did I feel that mistreatment was intentional but rather a product of their own high stress work environment and inability to meet "productivity goals". We all did a phenomenal job, but it was just never "enough". I wish the company well and hope someone in a power position there see's this and can enact meaningful change. You can have a for profit company and still have high ethical standards; you may survive much longer that way and make a lasting positive impact on the communities you serve. 

As a case manager, the case load was always high.  Some team members had to drive ridiculous miles to see patients.

I learned how to case manage a patient load.  I learned about death and dying; the symptoms, family teaching, and use of medications.  You have to utilize your team members.

Management in the nurse department varied.  But mostly, the managers were poor.  There was no help from managers when days were horrific, and they new it was that way.  They continued to send emails that we needed to call a patient, even though they were on the phone with the patient and could have helped.  the manager above the team managers was afraid of her team managers and did not stick up for the team members.  There was knowledge and proof a manager was targeting her nurses.  But anything the nurse did wrong, even any miniscule thing, was a reprimand.  There were a bunch of nurses fired.  some of the nurses were lucky enough to find different positions in the company.  And a few nurses quit without notice.  For being a hospice, this company was more into it for the money than the quality of its care.  Quantity, not quality.  Hospice needs to be quality, but that should not abused. It was disappointing.

Other than management above the nurse case manager, everyone was fantastic.  When you work in a job like hospice, you and your team mates become very close.  I literally loved all my co-workers.  I still love them. I see them out of work.  They are the kindest and caring people I have ever met. I cannot say 

There were a few areas that were difficult.  The charting was ridiculously long.  I never work 8 hour days.  13 hour days is really how it is everyday to make sure you meet your quota for the week. Not to mention having to chart on the weekends with no compensation. This is where the live to work people come into place.  On-call was the worst on-call programs I have ever dealt with.  The nurses were expected to work on-call at least 4 times a month.  Ok, that doesn't sound bad until you have to be on-call weekends and have to have your phone next to you for 24 hours. Yes, on-call 24 hours.  And yes, on-call for hospice is very busy.  At times, nurses were unable to get home and sleep a few hours.  The on-call pay was not enough. During the week, if you were on-call, you are on-call from 5pm to 8am the next day.  And if you were out all night, the nurses still had to work that next day.  The last thing was that you never had enough time in  the day to be with your patient and family due to quantity issues.  some weeks, unfortunately, many of your patients pass away.  I have seen up to seven in a five day period.  There is no way to help these families the way you want with the amount of patients you have.  

Unexpectedly, I really loved the nursing part of a Nurse case manager.  It is what nursing is all about.  full assessments, using nursing skills such as phlebotomy.  Almost anything that can be done in the hospital, we did in the patients' homes.  I know this will sound weird, but the teaching aspect of this position was very enjoyable.  I found through this, that the patients and families would bond with you through this.  You are helping them get through this process.  You are their "Google". You are the Nurse Manager of them, which means you need to know your stuff...everything from medications to signs of death. You are their counselor, a shoulder to lean on.  Meeting the patients and realizing how amazing they are and how strong they are.  They don't worry about dying, they worry about their loved ones.  This is what nursing is about...at least this part of it.

I love my patients and love providing PT for them in their homes.  The "real" environment that the patient's home provides is a challenge at times but you will really get to know the patients you treat....this is the best environment to see how the patient functions daily

1.  Incompetent management, money centered....lip service about caring for the patient. I will not be surprised when Medicare sues them for fraud.  "Well lets just make something up to stay in there" was a comment from second in command RN at weekly staff mtg.  When I first started working here  I was shocked at how thick the vital sign folders were in the patient's home...you know the one that most home health agencies require their staff to fill out. Well apparently Texas HH likes to keep re-certifying their patients indefinitely...it is crazy.  I've worked for over 20 years as a staff therapist and traveling therapist with numerous companies and have never seen this much evidence pointing to fraudulent recertification of patients. 

2.  Out of 20 pay periods they have shorted me 15 times.  15 times!!  Then when you ask why the boss will argue about the mileage ( their mileage policy is completely unfair) or they will say they will fix the problem and then you have to keep going back to get them to follow through with reimbursing you.  If you have one patient that is in Timbuktu they will pay you to drive there minus the distance from your home to the office and will not pay for the return mileage unless you have another patient in town.  They only pay 47c per mile here in Texas.  IRS rate is like 58c per mile...so they pocket your money.

3.  They hire new grads which is usually unheard of in home health.  I'm usually somewhere near the middle in experience when I hire on with a home health agency and I have 20 years.

4.  They hire foreign trained staff completely new to therapy or even home health .  This is a major headache.  Nobody can understand their accents  which is funny here in Texas.  Our patients and doctors think their talking to the help desk for AT&T in the Philippines or India.  Apparently the CEO works as a professor in California and hires his foreign students with a work visa scheme.  Seems like their is a conflict of interest there, right?

5.  All I can say is make this company the very last company you consider.  This company hinders their workers they don't enable them to do their jobs well.  They laid off their entire computer support staff after sending them all over the country to train the company promising them a position to continue with training the company.  They laid off other well respected employees as well.  Entire therapy staff at one office quit within 3 weeks of each other...kind of a hint huh?

6.  This is plain and simply a cutthroat company...they don't care about you they only see the money$$$.




Fire the CEO, all inexperienced management (20 years plus should be a requirement), get rid of the dictatorial management style, and foreign trained workers.  Invest in a better tablet software or maybe try Kinnser (it was developed here in Texas) and it is much easier.  Pay your workers, don't slice their mileage reimbursement or be chincy with your rate of pay.  You are opening yourself up to fraud with the cumbersome tablet documentation and the over abundance of recertification's of your patients.

I have never worked for a company that treated their employees as poorly as AccentCare. Corporate sends emails day and night. The worst part is that you can't get even a mental break from work because you wake up to these emails and with tasks that you suddenly now need to complete and with very little time before the deadline. Much of the info that comes out is confusing, not thought-out and erroneous so they end up recanting the info. Essentially, it is waiting for another surprise project to pop up and having not enough time to complete it. Everything comes by the element of surprise and last minute. The translation is that employees are working much longer days and on a regular basis, impacting the ability to spend time with family or have a break from all the stress. They are not open or respectful of our time or us as anything beyond human capital.

Everything is done with a profit mindset. Patients and employees do not matter AT ALL and the policies that get handed down from corporate in Texas reinforce this feeling over and over. Our local managers are completely powerless to help either because they are left in the dark or are told what to say which is repeating vague corporate promises that can be quickly reneged and taken back at a moment's notice. The change is dizzying. We have to attend weekly rounds meetings but productivity credit has been taken away. They split up cohesive teams to make business centers out of each office. The incentives like extra pay bonuses that Fairview had offered were taken away. Now it's even questionable whether anyone will receive a raise after the freeze last year during the pandemic. The work keeps increasing but any incentives that helped us along the way have been removed. Morale is very low with high turnover across teams because everyone feels demoralized and beaten down at our jobs. AccentCare has lost so many talented, caring people who actually wanted to do the right thing by the patients and many more will be leaving.

Run away fast from this organization if you want to be treated well and value your family/personal time. The loss of Fairview Home Care has been very sad because we felt appreciated and a part of something bigger and more meaningful. This has been ruined by AccentCare and its all-for-profit monolithic focus. There is no pride in that at all and people who really care about patients and quality care don't stay long or if they do, it's hanging by a shriveling thread.

All of this does not bode well for AccentCare long term. What goes around, comes around with how you treat others, including in business. Like others have said, Minnesota is a state with savvy patients and clinicians who will not tolerate shoddy care (and rightly so!), our nursing/therapy worlds are tight-knit because we network together and there are other large systems that will absorb our passion and talent. If a company wants to be so Machiavellian that the "end justifies the means," it might just come to its end a lot quicker and worse off than anticipated.

This is a for-profit home health agency based out of Dallas, TX.  
As a RN case manager, you will see upwards of 6 patients per day travel upwards of 99 miles per day.
Accentcare uses a point system to assign to patients that is  NOT based on patient acuity, but rather on whether a patient is a:  new admit, is being recertified for home health care (every ongoing patient has to be recertified for Home Health per Medicare every 60 days), or is what the company terms a “regular visit”. The point system is very flawed.   It is a way the company uses to determine if one is being “productive” and is a complete joke.  Again not based on patient acuity.
If you travel over 100 miles you get 1 point.   They will make sure your miles stay just under at 99.  They do not break this down into .25 increments(if you drive 25 miles you get 0.25 points and so-forth).
You will put wear and tear on your own vehicle.
You will be reimbursed 54 cents per mile when gas is around $5.00/gal.
You will use your own cellphone without a company stipend to coordinate all care related to patient, including calling patients to schedule visits and correspondence with provider offices.   You will receive calls and text from patients and provider offices after hours and on your day off.
You will work many hours after hours, charting, calling pts to schedule for the next day and returning calls.
You will be required to take on-call.   The compensation for on call is a joke.
It is quite impossible the way this company has the system set-up to meet their bottom line to give actual good quality care.   
You will be promised sign on bonuses and reviews with raises that this company will do everything in their power to not give you.
If you are needing refreshing of your bedside nursing skills, you will be told they have ample training opportunities that do not exist.   
You will get frustrated with the high turnover.
You will be scheduled to patients to complete tasks that you do not feel comfortable with and will constantly have to advocate for yourself and your license.
You will feel unsupported by office staff and supervisors.
You will plan for your day the night before, which means nothing, as clinical supervisors will constantly ask you to take on more.
You will love your patients and truly get an understanding of their social determinants of health in the home setting.   You will want to do more for all of your patients and get frustrated and quit due to the focus of MONEY, rather than quality patient care.
You will be required to “take” a lunch, although you rarely ever get one, other than stuffing a quick sandwich in your mouth while driving.  If you do not put a lunch into your payroll because you weren’t able to take one, this company will take money out of your paycheck to reflect that you “got” a lunch.
You will wonder why this company continues to admit new patients despite inadequate staff levels to provide care, and then you will realize, it is not about care at all, it is about MONEY.

I have worked in home health for greater than 5 years and this organization takes the cake for the literally the worse agency/organization I have ever worked for. In less than a years time I saw significant turnover in staff. The direct management team is highly disorganized and the rules seem to changed daily in terms of how things should be done. The communication was awful. The field staff was highly inept at doing their job well and patient care would suffer. Compound that with poor communication from office staff and it was a disaster in the making. Be prepared to set your route out the night before and call your patients and end up having more visits added to your schedule  the next day around midday without prior notification. I would spend around 3 to 4 hours just driving to attempt to get all the visits done for the day. The driving was very extensive with poor mileage reimbursement for all the wear and tear on your car.  I was promised a territory when I was hired but constantly was given patients outside the designated territory. Then it appears as though those doing start of cares and or marketers would promise crazy things to patients and their families like guaranteeing set treatment times and days without even seeing if that would work into your schedule. Not to mention the amount of patients that were picked up who were not even homebound per medicare guidelines. The agency uses Home Care Home Base for documentation which is literally  the most check box driven system in the world. It was ridiculous in how repetitive it was with asking the same questions over and over again. Literally the black hole of charting. This alone took away from valuable time treating the patient. Prepare to spend significant time at home doing charting which is a recipe for burnout. You will also  be micromanaged and bullied  from their regional administration staff. No-one seemed happy in their role and employee morale was subpar. Weekly case conference would be at best 2 hours long with clinical supervisor and executive director rambling with no set outline as to what needed to be discussed. Then sometimes they would call you the next day back in for another meeting to discuss something ridiculous in nature that they could have told you the day before. Way too meetings at this company in which you would think they could iron out all their issues which took time away from patient care.  Then they wonder why you end up with missed visits from all the incessant meetings they have about nothing.  Save yourself the headache and preserve your sanity and keep looking for somewhere that is stable and promotes excellent patient care and doesn't  look to disrespect, bully, and manipulate their employees.

Accentcare Fairview is a sinking ship.  There are many great staff members but they are leaving in droves.  Multiple people in different positions with in the company are resigning every week.  Accentcare management doesn't seem to care.  They dont seem to care about people, patients, families or employees.  The "old guard" and managers that were previously Fairview are doing the best that they can but have little to no power when it comes to the corporate money making machine.  Policies are changing daily with no regard to any of the above.  The benefits package is not great....and the benefits enrollment process is even worse. The Accentcare people are a mystery and unresponsive when being asked questions.  Raises are in question....there have not been any raise/increase for over 2 years for most employees and when management is asked what is to come in the future the answers are cryptic and unclear so likely no raises to be had.  They took a cohesive and competent group of people and separated them into isolated branches.  It seems this was very intentional in order to divide our staff.  Most employees have been grieving the loss of fairview hospice for awhile.  When Accentcare came into the picture everyone had said "it can't get any worse" but apparently they were wrong. Very wrong.  The hospice director cannot be trusted and will drive this program into the ground with the help of Accentcare leadership.  There is zero transparency.  Promises are constantly being made and broken. It appears management is being coached in talking points.  There is no attempt to retain staff yet they say staff retention is the current goal/mission. Accentcare has not gained any fans in the midwest market. I WOULD NOT RECOMMEND THIS AS A PLACE TO WORK FOR ANYONE!!!!!  Choose ANY other hospice, choose a place that cares about its patients and its employees....As many before have said RUN dont walk.......I am not entirely sure that this company will be able to survive here as they continue to lose their best staff and the places that use them see that staff are leaving at an alarming rate.  And on top of that the care will start to decline if it hasn't already as the focus is to get them in but not necessarily care for them once they are in.
This is not a company that anyone is proud to work for at the moment.  The reverse in fact......RUN RUN RUN

The transition from Fairview to AccentCare Fairview has been something to be desired.   Communication is scant and often conflicting.  Oftentimes information is sent last minute with demands that are unreasonable and unrealistic for those of us in the field and tending to busy schedules day in and day out.  Or, information is sent, and then quickly retracted.  

Also, employees who work less than .8 are no longer offered benefits including time and a half when required to work holidays!  No benefits are provided for weekend work, either.  Productivity no longer includes the many demands of good case management so be prepared.

There has been zero interaction with our new Accentcare coworkers except this strange "wizard behind a curtain" impression every time a new edict comes through the grapevine.  No decisions are made from within - everything goes to some mythical place where no one seems to care that there is no staff, no communication, and no clear direction.

While managers have these new fancy titles, it is a title only.  Very little can they do.  And very little do they even know and so if you send an email to your manager, be prepared to NEVER get a response.  It seems easier to pretend not to acknowledge an email than to admit that Accentcare has given zero direction to leaders on how to properly direct our large organization. 

I get the impression, in "other markets," Accentcare tends to push its employees around with little regard to employee satisfaction, retention, or concern.  That just won't fly in Minnesota.   Minnesotans, by nature, are hard workers and don't need every training to begin with:

* We are watching you.
* Your manager is getting updates on this.
* We may come back to you if x, y, z.

Also, the additional hoops to jump through to get insurance are simply ridiculous!  There seems to be general mistrust between employer and employee which is very sad, indeed.  There is this feeling like another shoe is about to drop every day.  It is the worst feeling. 

All in all, Accentcare is over its head in Minnesota if it thinks it will attract and retain qualified employees by treating its employees like chopped liver.  They need to start treating us like the professionals we are.

This is place is terrible. When I first applied seasons told me I got the job for one position and then said that it was a mistake and forced me into another position that I clearly did not want. When there is nothing to do we have to do home care visits for the NCM which is fine but they give us like 8-9 patients to see in one day after you have been sitting in the office for 2-3 hours waiting to do something. We are supposed to do continuous care for patients who are symptomatic but we don't have enough staff cause the nurses either leave or get fired for crazy reasons. They don't consider LPNs as nurses, they call us the "LPNs". When we don't have any continuous care cases they force us to go home early using all of our PTO or they make us sit on the inpatient unit and sit there and tell us to wash patients up or what ever the RN tells us what to do. Sending us out to do CNA/GNA visits to wash people up all day. Then they may call you and say that they need you to start a case and you are supposed to stop what you are doing and start that case.Management constantly changing our work schedules cause staff members are leaving making us work days and nights in the same week and saying that's what we were told when we were hired which is a lie. If your manager does not like you they keep doing things to you so you can quit. For example, give you a schedule that is ridiculous and unfair. My fellow CC nurses are great we all got along just fine. Showing favoritism to your other staff members by letting them switch to another day that is good for them. You can not go to HR about anything they automatically will help the manager in any situation. My co workers (CC nurses) are great even though I have seen 3-4 of them go w/in months to less than a year of me being there. The best thing is taking care of patients. Manager of continuous care are very rude and are liars. If you do what they say and don't say anything then everything will be fine but don't speak up they don't like that and will target you. STAY AWAY! ESPECIALLY LPNS!
