Typical Day at Work:
I am a care coordinator for those families and individuals impacted by the opioid epidemic that has ravaged the isolated communities of northern Wisconsin.  My service area spans three large counties so transportation in all types of weather is challenging for both myself and my clients. On a typical day of work, I might meet with clients and those supporting them including counselors, doctors, probation officers, and individuals from the various agencies across the northern region that can provide help to both the afflicted and their families.  As the job title suggests, I then coordinate their care.
What You Learned:
My clients come from all backgrounds and social status.  Case by case, individual by individual, I have learned to embrace the wins and prepare for start overs that inevitable are part of dealing with addiction.  I have seen the terrible toll that addiction takes on each of them and their families.
Management:
The care coordinator position as designed to deal with those impacted by the opioid epidemic is relatively new to both northern Wisconsin and Marshfield Clinic.  Over the last four years I have had three different Marshfield Clinic supervisors all of whom have been very supportive of the position and the program. I believe that Marshfield Clinic is committed to making the program a success.  They have provided resources and seem willing to listen and act on care coordinators concerns and recommendations.
Workplace Culture:
Family friendly.  Two years ago, my daughter was diagnosed with a life-threatening medical issue.  Through more emergency visits than I can count and multiple surgeries (three in the last three months alone) Marshfield clinic has stood by me and my family.  As a single mom it means the world to me to have both supervisors and an organization who understands and will listen and provide support in your time of need.
Hardest Part of Your Job:
On a winter morning when it is twenty below zero and they are predicting two to four feet of snow I might be tempted to say the weather and the upcoming drive.  In reality, it is seeing the terrible toll that addiction takes on families.
Most Enjoyable Part of Your Job:
The Wins:  A family getting their children back.  Clients getting their first job in years.  The pride that comes with a driver’s license, a car, an apartment to call their own, starting college, to name just a few.

Loved my job and learning from ALL co-workers and management. I am a very hard worker and always want to do the job correct and make it a career. Misleading as to fact that, "most likely it would work in to a permanent position,you never know", which it very well could have.  Should have given more time to learn all aspects, due to fact that I had no "clinical" experience, but had ALL the credentials and education and more, and was completely upfront and honest about this fact that I had no "clinical" experience in this particular genre. They wanted to hire me, despite this fact. I did VERY well, but was not given long enough to develop all areas. We all work at different paces. My Director complimented me and said "you NEVER let me down", quote-unquote.  There was one particular employee who was brilliant and a true asset to the day-to-day performance of duties, etc. I liked working with this person and learning from this person (if and when this person allowed me to), as I liked learning from Everyone. That is the type of person and worker I am. I like working as a Team and learning from everyone and I respect everyone. This particular person, no matter what, did not feel the same, nor return the same behavior(s) or work behaviors. The work environment was "toxic" because of this person, and that was NOT just my words, it was words of others who had worked with this person, unbeknownst to me at the time.  Apparently, no one would say anything.  I overheard one co-worker say that this person wanted everyone to "fear" them. I continued to respect this person and try to learn from this person.  This person liked who they liked, and unfortunately, talked about others behind their back, even about the Manager and Bosses, who, without these Managers and Bosses, this person, nor Any of us, would have jobs!  I COULD have, one particular day, filed a personal harassment complaint about this person, but I did not and am not that type of person to do such a thing. I prayed for this person.  It is unfortunate that these types of scenarios are allowed to occur and continue to occur and that the person does not wish to change or even look at themselves to realize that......no one is indispensable.
I continue to support Marshfield Clinic, as there are many departments that are wonderful.  This particular department could be much more. I did appreciate working there and the opportunity.  I continue to pray for this person and will not judge. Only stating truth and facts.

True leadership is lacking. What this means is that there is a lot of micromanagement, but no grand plan for the future direction or development of the clinic. The board spends an inordinate amount of time trying to dictate how providers schedules should look, how staff should appoint, how much money providers should make, how large provider panels should be, how many meetings providers should attend, how vacations should be doled out, etc. BUT, they spend NO TIME trying to figure out what the clinic should look like in 2, 3, 5, and 10 years! The job of a BoD is to figure out the future growth and business strategy of the corporation.  It DOES NOT bother itself with the day-to-day workings of the individuals at every level of the organization.  THAT is what you have managers for! The lack of a strategy is frustrating to everyone.  If you are a provider and you want to develop your own personal plan of development, you are hindered by division administrators, who feel the need to dictate how to practice medicine and who loathe providers with any vision for their own professional and practice growth. Communication skills are non-existent.  Employees only found out layoffs were coming when it aired on the local news, NOT from administration. And even when administration decided to announce it, they did it by email. Even a place considerably larger, like the Cleveland Clinic, lets their employees know through a meeting where the CEO is physically there.  Not some generic mass email to everyone in the clinic. The way things are currently run the clinic is in a constant state of panic. Morale is low, but there is nothing being done to make it better.  Instead management spends millions on new signs and furniture, then turns around and asks people to participate in a "voluntary" severance program, before then just going and laying more people off.  The end result is that the employees are traumatized and constantly looking for a way out of this place.  The good people are leaving in droves which is making things so much worse.

The providers are absolutely wonderful.  I can’t say enough about how dedicated they are to their patients and how nice they are towards staff.  Unfortunately, the head honchos only care about money and have made patient/provider relationships into a fast care drive through type of scenario.  The big wigs don’t care about you as a patient.  The hardworking providers and staff do but they are overbooked, overworked, short staffed, underpaid, under-appreciated (or should I say never appreciated).  Christmas bonuses were taken away from us.  Our Christmas party was taken away.  The clinic matching a nice percentage of our retirement was taken away.  Our insurance, once amazing, is now absolutely terrible.  Staff gets screamed at every day by upset patients and managers don’t care or try to fix anything.  We are threatened and scared and angry patients walk through our doors yet we can’t get locks on the waiting room door or windows for secretaries.  Patients will walk right into your office and up to your desk.  Locks and safety measures should have been implemented after an incident a few years back.  Look it up online.  The providers, nurses, and secretaries get blamed for the big wigs poor decisions.  Recently, several providers who sat high within the clinic dynamics, stepped down because they knew this place is falling apart.  The turnaround in this place is unlike anything I’ve ever seen due to so many reasons.  Terrible training if you get any at all.  Most of the time a brand new staff member, fresh out of school, is thrown in there and is expected to survive and do their best because someone called in sick and multiple people call in sick every single day because they hate working there.  If you work in specialty departments you may be pulled to work in another department.  You may even be pulled from your own provider to work in another department.  No incentives-NONE.  No positive feedback from management.  Why would someone want to work in a miserable, toxic, and absolutely stressful environment?  

I am in a position where I care for patients and am autonomous in that care. The staff are clearly overworked to the point that if I need something I am unable to do my job appropriately because they are having issues doing theirs. If one system isn’t working well it brings down the rest and patients suffer. I am a college educated person and I will research code violations and it is VERY clear that there are many violations structurally and administratively. There are buckets that catch the water from leaks in the ceiling. There are wood studs being used for building extra rooms on certain units. Fire detectors covered with tape so they don’t go off during maintenance. HAZMAT patients are being hosed off in the ambulance garage and it is being drained into city systems. Covid patients are being placed in units where immunocompromised patients are. Not all staff members are being told to garb up for Covid patients even though they have to be within infection distance. Staff members are being reprimanded for mentioning any of these issues. Managers and those above are being let go without replacements so there is no direction for staff. Medical records have been outsourced and have caused MAJOR issues with HIPAA violations. There have been numerous racist comments stated by a main building service manager (Central). This company is great at removing those that actually care and want the best for patients and staff while they keep cutting budgets in order to pay the CEO’s. The consulting services they utilize have cut so many departments and budgets to the point that some uneducated consultants are being put in charge even though they subcontracted to “consult”! 

Marshfield Clinic has fine physicians and a growing practice but they have a great flaw and that is they want to be Mayo .

The decisions made through this system are very political and almost never good for business. A number of years back Marshfield Clinic was successfully sued for trying to monopolize a region so it responded by having a culture of "running everything through legal" system. It is great if you are the lawyers... I was not. The governance system makes the clinic very uncompetitive with other systems of size; they also have a tendency to drop programs rather than fixing them.

In my 11 years of health care, my day consisted of reviewing my schedule to see how many patients I was going to be rooming for the next day and completing each patient's chart per the Doctor preference and requests. I would set the exam rooms accordingly to what the patient was coming in for. For example, if a patient was coming in for a wart treatment, I would make sure the liquid nitrogen container was available for the doctor to use to treat the wart. Once I called the patient back to the exam room, I would wash my hands and then review patient' medication, allergies, prevention reminders and note their reason for visit. If patient was due for any vaccinations, I would complete that for the patient per the doctor's orders. If any lab work was needed, I would put that into the clinical order manager per doctor's order. After patient leaves, I would go back in the exam room and wipe down the exam table, seats and counters for the next patient. 

My managers have been the most supportive to me throughout my 11 years. Their doors were always open for any questions or concerns. I have grown to be a better person and employee for the team because my managers were wonderful. 

What I enjoyed most about my job was seeing my patients. If they came in upset, I made sure they would leave feeling happier. It was so important for me that I made sure I do my part because I would want to be treated like that too. 

The hardest part of my job is seeing patients sad when they hear bad news about their health. I have learned in my career to empathize with the patients and let them know I care and although I am not in their shoe, that I will carry their burden with them.

What is the best part of working at the company?
Left alone by boss for the most part.

What is the most stressful part about working at the company?
People within organization had zero clue about supply chain issues created by COVID. Then when an explanation was given they refused to believe you. A large percentage of people in building would lie and throw anyone they could under a bus to make them look better. I caught so many lying via email and confronted them and their boss's. Nothing was done. A very small percent of people in that building are "good" people. Literally the most two faced place I've ever been part of. I also have a deplorable story about an experience about a medical visit I had personally with clinic. Long story short they tried to get me to do a sleep study at a cost of $6000. After they told me this was my option, I asked about a home study. They said it was available for around $500. When asked why this wasn't given as an option. I was told we only offer if asked. This is borderline criminal in my opinion. If they do this to an employee, I wonder what is done to those who aren't.

What is the work environment and culture like at the company?
Brick and mortar environment is fine. People and culture are toxic. 

What is a typical day like for you at the company?
Go about my day, hear gossip from people in all departments in passing. Most believe they are the most important part of the building. Looked down upon as if you are not on their level since you don't work with patients. No comprehension of economics of their departments as I was their supply chain provider. Not exaggerating on that in least bit.

In the past three years I have spent time working the the medical field as a medical assistant. My daily duties were to prep patient documents prior to their visits. Other tasks were phoning patient's with results, taking their vitals during a visit, reviewed their health history and medication lists. Help the provider with minor procedures, give injections when necessary, order labs, set up medication renewal, and handle faxes and other paperwork.   Prior to that I had worked in manufacturing for 16 years. These two jobs are totally opposite from each other, but both have given me many skills and work experiences that have challenged and fulfilled me. The hardest part of my last job was to learn to adapt to the continual changes in policies/protocols and not having supportive management. The best was to adapt for me was to make sure I understood the changes by attending the meetings and reviewing the policies/protocols as they were presented. My most enjoyable part of my job was the location and meeting new people on a daily basis, building a relationship with them, and looking forward to helping them.  I have learned that any job will have challenges to face. Its how you face those challenges that will determine your success. My approach has been to continually work hard, keep my focus on my job, and try to keep the communication open with management and co workers.

We were constantly changing how things were done. When I started, we worked for a center and did those doctor's first. If another center was behind, we would then help them to get caught up. We then went to specific doctor's, different centers. We had so many changes I don't remember them all. When I left, we were in Specialty Pods and transcribed only that specialty. To detail what I learned would take pages. The body is very complex. With the host of diseases, cancers, and even the common cold, diagnoses and treatments are changing constantly. Management was dependent on who you had and when, what department they were coming from or if it was an outside hire. You did what you were asked and didn't "rock the boat". My co-workers, for the most part, were amazing. The hardest part of the job was the day to day struggle of figuring out what some of the doctors were saying. Whether it be their dialect, slurring, talking too fast, eating while dictating, burping, yawning and talking, or dictating from home and kids yelling, crying, playing band instruments in the background, we had to figure it out to the best of our ability. I did enjoy my position with Marshfield but with voice recognition and the implementing of this as a pilot in one of the specialty departments, I decided it was time to pursue my first love, and that is art.
