Transitions Hospice

I, personally, would not recommend Transitions as a place of employment.  The employees are not valued, in my opinion.  Leadership is poor and unorganized.  The environment is chaotic.  .  Communication is poor.  Employees are frequently upset and disgruntled.  Significant changes are made frequently without thought about how they will impact the staff.  These changes include changing an employee's role or work hours/days abruptly without considering the impact that this will have on the employee or the employee's family.  Employees are not encouraged to grow in their professional roles, or to contribute their skills and knowledge for the betterment of the company.  This company was set up with some of the most intelligent, knowledgeable and experienced hospice professionals in the Indianapolis area, but for that reason, those clinicians have been pushed out the door one by one.  Their knowledge and experience is not needed, nor wanted.  You don't need smart people when ultimately, you don't want their input. These employees could not be moved out of the way any faster. 
 In fact,with employee after employee submitting their resignation one right after the other, one would thing that the ownership/leadership would question what is going on.  Nope!  Nothing.  It was as if this is actually what they intended to happen.  Not one question or comment when resignations were being turned in.  I would have at least expected something, maybe, "Hey, I am sorry to see you go", or "Is there anything we can do to change your mind?"  Nothing was said...crickets...Another significant issue at Transitions is micromanagement.  This significantly decreases job satisfaction.  In fact, so much time is spent micromanaging everything that everyone is doing, that other very important things are being overlooked.  Employees do not feel trusted because of this.   Dictatorship is truly the leadership style.  Some of the "leaders" that come from the Illinois branches seem to have ulterior motives.  They lie, manipulate and thrive off of the drama.  If an employee were to come to one of them with an idea, they would turn it around and make it as though it was their idea.  It seems as though the goal is to make others appear incompetent and incapable, while at the same time they desire to make themselves look heroic in their efforts to "fix" all the issues.  Ultimately, I feel that the people who do these types of things have a very low sense of self worth.  They thrive off of the praise and recognition that they get from the ownership.  The technology that staff are expected to use is way outdated and is in poor, used condition.  Employees do not receive mileage, only a car allowance which generally isn't enough to cover the miles that they drive, gas and wear and tear on their vehicle.  It's a sad state of affairs.  Save yourself the headache, drama and heartache and do not become employed with Transitions.

Anyone who loves hospice might embrace the concept and be excited about this opportunity.  But don't just kick the tires, look under the hood closely.  Ask tough questions and ask about how many former employees have left and why and monitor the reaction.  Look closely to see if there's defensiveness and the blame game.  Ask yourself if there's a reason all those nurses left.  Ask to interview with the staff alone. This may be a huge leap and you want to have a sense of the culture and support for ensuring your success in the field, is an important component.  
With all that presentation --which can be a slick sales pitch--be prepared the delivery is questionable and the way staff are treated is deplorable.  They may beef up the poor training and the onerous call schedule and hire a local manager that they don't currently have, but....upper management is arrogant, condescending, dismissive of ideas, mocks people behind their backs and will downright dress you down with an aggressive barrage if you ask a legitimate question that they feel is threatening.  If you question some practice you will be told, "you just don't understand the hospice concept." 
Most don't begrudge anyone making a profit but you will be treated like a dog if you calmly discuss facts that don't add to the profit margin.
As a RN with experience, I refuse to be an apologist for organizations that talk a big game but lack basic fundamentals, thinking rapid growth is a fix.  Don't confuse growth with thriving.  There are some good people working there...but sadly they aren't in control.  
Finally, the benefits aren't competitive...40 hours of vacation a year isn't even close to standard. United Healthcare for insurance is high deductible.  The 401k match varies on what the owner wants to do year to year. $200/mo for car may not cover gas/wear and tear, given that the coverage area is wide and they send RNs to every call no matter the reason. 
Only take the position if you don't have anything to lose.

I do not recommend this company at all! They do not have direct deposit, so good luck with getting it in the bank on Friday. There is absolutely little Management or support, and I drove over 500 miles in 3 days! There is a very small gas stipend of 92 per pay period which barely covers what is spent. You are not compensated per mileage, and you are not given any stipend for wear and tear on your vehicle. There are constant changes, with very little communication. If it is your weekend to work, and there isn’t a night shift nurse available, you take call. Sometimes, you work all day and night, and your management expects you to be ready to see patients the next day. The turnover rate is VERY high! The interview process is very “cushy” and they pad the job and responsibilities just to hook you in. Do not fall for it. Do not try to question management, or talk about fairness, because they’ll tell you to deal with it or find another job. The level of professionalism in this company is very low. There is absolutely no structure, and very little support. The training is absolutely terrible as well. I had very little training when hired. The benefits are dismal at best. The insurance is very expensive for little coverage. The paid time off is very minimal at about 1 hour per pay period. This is well below most area hospitals. Lastly, there is little room for growth, because management will create new positions and just promote who they want for the position. There is no formal application process amongst active employees to give fairness to those who may be interested in the position. I would not recommend someone to work here. This company is very understaffed for the demand. Just stay away!

RN Case Manager at Transitions Hospice. Huntley area location. Had prior hospice experience as RN prior to hire at Transitions. Outstanding quality to patient care. Quality to patient care never lacked despite periods of increased growth throughout company. Patient focused care and dedicated staff 24 hours. 

Staff is dedicated to working in team approach. Weekly interdisciplinary meeting held weekly and are productive and great learning opportunities.  Excellent hospice medical director, physicians, and NPs that respond within minutes to calls, text, emails helping create care plans and treatment orders. Medical team is available for coaching and development of hospice knowledge. NPs have made joint visits to patient homes with me in challenging situations. Acknowledgement from management when visiting corporate office. Hospice CEO, directors, and company leaders make effort to say hello and ask how things are going. Over the years I have participated in meetings with managers who have invited me into office for feedback on ways to promote ways to improve care in the field. Positive and negative feedback was always received professionally. Improvements I have observed over past years has been introduction of the extended care team with dedicated night clinical staff that greatly relieved the daytime clinical staff of heavy afterhours responsibilities. Work life balance improved greatly with this change. As of now it is very rare to be called out for work after hours. If so, pay scales have increased allowing for fair compensation of additional hours worked. My requested for PTO / Sick time have never been an issue and coworkers and nurse managers have been very understanding for need for vacations and time away from work.

Transitions Hospice is growling quickly because of the great business plan, culture, and  true desire to help patients by providing the best care possible. The company's growth provides a secure employment opportunity, my job will never go away.  Growth also provides significant opportunity for upward advancement and mobility.

The company was great during the COVID crisis. They allowed working at home and flexible hours to accommodate various family personal issues.

Senior management is accessible, listens to its employees, is accommodating and truly wants each employee to succeed by providing tools and state of the art technology.

There is great job satisfaction.  We definitely provide a needed service in the hospice community. Many hospice companies have a facility where they wish patients will go to pass away.  Most people would prefer to pass away at home with loved ones and family surrounding them instead of at a hospital with doctors and nurses around them.  We provide 24/7 care in every persons private home.  We get to know the families of our patients and provide total emotional support for the entire family. We service all people and can support any religious or ethnic customs the family may need. It is a truly personally gratifying experience, knowing that we make a difference in peoples lives.

PROS: I can not think of any

CONS: NPs report to RNs. No autonomy. Approval by RN mgmnt communication conflicting, 

Lunch rare & if happens occurs while driving, 

Shady practices whispers. Told mgmnt pt suited for pallative told to put them on hospice anyway,

Owner knows everything-only listens managers. Fast talker out of touch w/ ground level employees,

No mileage reimburse fuel card only. Expensive benefits, 

Always understaffed existing staff overworked & afraid to speak up,

Ineffcient workflow,

Unrealistic demands w/o regard/respect for time off,

This company does not care about any of their staff whatsoever. They just simply see people as a body covering a shift. They hold their staff to unrealistic expectations with absolutely no guidance, assistance or staff members. Corporate will micromanage your every move and also complain about lack of communication but will not communicate any changes/updates/new hires to you until you see them for yourself. You will have no free time or life outside of work as they simply do not care. Towards the end I was working here it was hard , almost impossible to have anything ordered for this company such as toner ink, medical supplies, DME, etc because they do not pay their bills. Facilities started to call and complain but nothing is ever done. Corporate will act interested in your concerns and will sit and lie to your face but nothing ever changes or gets done about any concerns. No one is ever in the office. Many times corporate will ask you to buy $400-$500 lunches for office parties out of your own pocket, claiming they will expense it but never do! Just so many things wrong with this place. It’s sad for the ones who are in it for the right reasons. If you choose to work here, goodluck! 

Transitions is not a place for an average employee seeking a ‘job’ to ‘pay’ the bills who needs micromanaged in order to be successful.  Transitions is a company in which personal discipline and professional growth is a necessity.  It will only be a good person/job fit if the employee is passionate and capable of  embracing challenge, refinement, and autonomy.  Many companies exist where one can punch the clock and be done with their day within 8 hours; leaving empty, frustrated, and searching for a ‘bigger purpose’. Transitions creates purpose!  It is a place that demands personal conviction and a selflessness of always doing the right thing.  It requires a special type of devotion and commitment to serve others.  This company served as a launching pad  to serve beyond myself and to see a ‘bigger picture’....to fully understand this world does not revolve around me, and that I can make a difference!  This is not a company for the ‘faint of heart’,  but rather a company for those who are seeking fulfillment, purpose, and significance.

What is the best part of working at the company?
Poor pay and no travel or auto reimbursement. Lots of turnover in management and staff. Medical providers and support staff amazing and care about patients, upper management all about money and  reimbursement. Payed upper tier large salaries when the people on the ground who were actually bringing in the money by seeing patients where poorly compensated. Little field training and follow up poor

What is the most stressful part about working at the company?
Many hours of uncompensated time with charting and phone calls. Having notes and all paperwork submitted in a short period of time 

What is the work environment and culture like at the company?
Providers great and knowledgeable, management and owner terrible with no communication and unanswered messages. All about money. Tells the same sob stories over and over, wasted times with meetings

What is a typical day like for you at the company?
Leave home at 8 am, see patients until 3 pm then and submit referral until 8 pm.

I started in June, NEVER have I worked for a place that has its Patients AND Employees as #1 on their list, This job has forever changed my life in the work field, I can go visit a patient and actually TAKE THE TIME I NEED to provide the BEST care, comfort and companionship to those who only have a small amount of their time left here, The nurses actually get in and do the work with you!!!  Everywhere I've ever worked once a CNA gets their nursing license it's like they're too good to help anymore but not here, the nurses work just as hard as the CNA's it is absolutely all teamwork together and I think that is absolutely fantastic!!!  I like how you have the freedom to be able to go visit which patient you would like to start off with for the day, And be able to spend as much time needed with them, It's never a rush rush situation because transitions puts the Patient 1st always!! This here is my Dream Job!!! The staff has always been so good and wonderful with communication or helping me if I needed them!!! All I can say is you won't regret working here at all!!!
