I have been with Transitions LifeCare, founded as Hospice of Wake County, for 25 years.  The strength of the company is our people, our teams and our values.  The reward for what I do is the knowledge, the grace and the blessings we get back from patients and families that we serve.  
My vision is to increase the utilization of hospice and palliative care. At the end of the day I want to make a tangible difference in people’s lives one patient and family at a time.  Helping each patient navigate the end of life is our mission.  
What inspires me every day is the commitment of our staff and volunteers, specifically their commitment to quality and being the best at what they do.
We do tough work and work hard.  The work culture at Transitions is supportive of each other and our core values are very much alive.  The compassion, respect, teamwork and commitment are palpable.

A typical day as a Nurse Liaison was an ever changing opportunity to provide excellence in care and coordination of care with patient's, families, hospitals, doctors, nurses, case managers and others instrumental in achieving the desires of the Hospice Patient.  I learned how to suspend my agenda, perceptions and judgment to successfully meet the patient and family where they are during a most intimate and dynamic time in their lives.  I learned that compassion and caring along with honest explanation of how Hospice helps and what Hospice really is helped me to expand in my professional and personal growth.  Management was the perfect balance of hands on - hands off.  The job gave me the education and autonomy to use my skills and talent in my own way.  The culture was open, caring, supportive and had a much appreciated transparency.  The most enjoyable part of the job was of course the special connection during a very special time but also how perfectly the job allowed me to have a rich and healthy personal life as well.

When I was hired a this position I was trained to do the job. I have learned a lot since then, however, part of that work involves my dedication, compassion toward the families, and my sense of work ethics.
Typical day at work: 
Check your workflow from the day before, check emails that pertain to you or the patients, answer calls, work up a chart to start a referral, work with RNs to coordinate patients care, perform tuck in, transfer of patients from one hospice to the other, or visiting patients.
Management only cares about the job you do and how well you do it.
Workplace culture is ok.
The hardest part of the job is that it drains you
The enjoyable part is the PTO and benefits.

Transitions LifeCare is a wonderful place to work! Mainly because of the warmth of the employees. Yes this is a place where end of life care is the main focus; however, it is care given with warm intentions. I have never worked at a place where people are always smiling. Even those nurses/doctors dealing with death on a daily basis are joyous souls.

This is was my first job in dealing with any type of healthcare. A part of the management was difficult, as one of the managers was unsure of exactly what her role was. She was in places and giving directions that she should not have been.  Nevertheless, a great job.

What is the best part of working at the company?
Meeting and helping patients and families.

What is the most stressful part about working at the company?
Limited resources for critical situations at times.

What is the work environment and culture like at the company?
At first, it feels as if you are working toward a more wholesome and compassionate cause. But then it becomes clear that priorities are about how to make money.

What is a typical day like for you at the company?
Mostly in the car and coordinating care or in patient homes and comforting whomever needs it.

Transitions Lifecare in Raleigh, NC is a not for profit organization.  This has its ups and downs.  Overall, I love my job and the people I work with.  I have to move and will miss it so so much!  Staffing is down like every other healthcare place these days but setting boundaries, they don’t hassle too much or make you feel guilty.  They always figure it out but like most NFP places, the numbers matter, sometimes too much.  The facility is beautiful and our patients and their families really need this inpatient facility.  If I could package it up and take along my coworkers, I would.  You will love it here!  

Client visits with the TCC program. The manager and assistant manager were very hard on the nurses. Was told I could not sit at the desks in the office to answer phones???? Very stressful environment and worked with a social worker who was very condescending to the nurses. The best nurse I worked with is now retired and my friend for life, but that does not help the work environment now.  

excellent care given to the residents, family and friends whom they  treated with great respect as well as if they were their own family.
the best part of this job was we were a family,working side by side and always there for each other.the  most enjoyable part of this job was my contact with residents, family and friends.to lend support or to just listen my greatest joy is to know i made a difference and/or brought a smile to their lips. there isn't any hard parts to this position. there is alot of concern for the families the last time they go out the back door where the hurste is waiting. all in ll this is a fantstic place to work

Company is focused on meeting financial and numbers driven needs more so than common sense practice and quality service to its patients and staff. Overbearing micro management culture with frequent bullying tactics employed by managers. There are a number of caring and high quality support staff, nurses, chaplains and social workers who work there but are under the constraints of poor management and leadership. A lot of staff turnover as a result.

What is the best part of working at the company?
Flexibility supervisors are very understanding . Work from home days can be flexible .

What is the most stressful part about working at the company?
Sometimes your conversations are shared . With at least the supervisor I had she trusted the wrong people

Multi-cultural everyone is welcomed no discrimination detected

Tedious and long a lot of busy work
