Monitor patient's ED visits and acute stays, perform post-discharge follow up calls and continuously assess risk of readmissions post-discharge.

Receive and conduct medical phone calls along with appropriate documentation. Place referral calls when requested by NP/PA/MD. High School Diploma or GED.

MSHA certification is required to work in quarries, gravel pits, construction sites, and aggregate operations. Job Type: Full-time with overtime.
