Care Navigation Facilitator

Schedule: Monday–Thursday 8:30-5:00; Friday 9:00 a.m.–3:00 p.m.

Work Week: 38 hours paid as 40

Work Location: This position is primarily on-site, but employees must have a dedicated space at home for a remote work set-up to be used on an as-needed basis.

Position Overview

Are you passionate about helping people navigate healthcare with clarity, compassion, and confidence? Liberty HealthShare is looking for a Care Navigation Facilitator to support members as they explore healthcare options, understand billing practices, and identify high-quality, cost-effective care resources.

In this role, you’ll serve as a trusted advocate for assigned members, guiding them through healthcare sharing education, provider participation, fair and reasonable pricing, billing processes, and available healthcare resources. Your work will directly support informed decision-making, responsible stewardship, and exceptional member experience.

What You’ll Do

As a Care Navigation Facilitator, you will manage assigned cases from start to finish, serving as the primary contact for members throughout the care navigation process. You will:

· Educate members on Liberty HealthShare Sharing Guidelines, fair and reasonable pricing, balance billing processes, and provider participation.

· Communicate with members through phone, email, and written updates to support timely case resolution.

· Research providers, facilities, and healthcare resources using approved systems.

· Help members identify participating providers and cost-effective care options.

· Coordinate with strategic vendor partners offering bundled pricing, specialty programs, and alternative care resources.

· Perform cost comparison research, complete pre-claim estimates, review member estimates, and educate members on expected pricing and billing practices.

· Document case activity, workflow milestones, member interactions, and outcomes accurately and on time.

· Collaborate with Care Navigation Nurses, providers, facilities, internal departments, and vendor partners to help resolve cases efficiently.

· Identify and escalate complex member, provider, or operational issues when needed.

What We’re Looking for

· High school education or equivalent.

· Microsoft Office and Excel proficiency.

· Ability to interpret healthcare billing documentation.

· Strong organizational and prioritization skills.

· Knowledge of medical terminology, CPT coding, and ICD-10 coding.

· Excellent written and verbal communication skills.

· Revenue Cycle Management experience.

· Background in a clinical setting.
