Key Job Responsibilities

The Regional Social Worker plays a key role in supporting dialysis patients across multiple clinics and skilled nursing facilities through comprehensive case management and care coordination.

Responsibilities include:

- Conduct biopsychosocial assessments and ongoing psychosocial evaluations in accordance with dialysis regulatory requirements.

- Perform depression screenings and psychosocial follow-up, ensuring patients receive appropriate mental health resources and support.

- Provide comprehensive case management and care coordination for dialysis patients.

- Coordinate transportation resources to ensure reliable access to dialysis treatments.

- Assist patients with insurance navigation, including Medicare, Medicaid, Medicare Advantage, Marketplace plans, and Medigap coverage.

- Support grant writing, applications, and management, including programs such as the American Kidney Fund Health Insurance Premium Program (HIPP).

- Facilitate transplant coordination, including communication with transplant centers and tracking patient transplant referral and evaluation status.

- Develop and support initiatives that promote patient wellbeing, including programs focused on treatment adherence, mental health, and social support.

- Collaborate with nurses, physicians, dietitians, and clinic leadership to support interdisciplinary care planning.

- Maintain accurate documentation and patient progress notes in the electronic health record.

- Connect patients with community resources addressing social determinants of health such as housing, food access, counseling, and financial assistance.

- Support patients receiving care in skilled nursing facilities and coordinate communication between facilities and dialysis clinics.

- Preparing and presenting monthly Quality Assurance and Performance Improvement (QAPI) meetings

- Completing initial, 3-month, and annual care plans for patients

- Rounding with nephrologists, dieticians, and registered nurses

Position Structure

This is a hybrid regional role supporting multiple care locations.

The Regional Social Worker will initially support:

- Two in-center dialysis clinics

- Two skilled nursing facilities

As the organization continues to grow, the role may expand to support additional sites and initiatives.

Qualifications

Required

- Bachelor’s Degree in Social Work (BSW), LMSW preferred

- Active Texas social work license (or eligibility to obtain one)

- Reliable personal vehicle that can consistently and safely support travel between clinics and facilities

- Strong interpersonal and relationship-building skills

- Excellent organizational and time-management abilities

- Ability to manage multiple responsibilities across sites independently

- Comfort working in a fast-paced healthcare environment

- Comfort with technology, such as drive, EMR, and internal communication tools

Strongly Preferred

- Licensed Master Social Worker (LMSW)

- Bilingual (English and Spanish)

- Experience in dialysis, healthcare case management, and/or social services

- Familiarity with insurance navigation, patient assistance programs, or transplant coordination
