Compensation: $75,000-$110,000 Annualized + Incentive Visit Opportunities
Schedule: Full-Time Monday - Friday | On-Call & Weekend Rotation Required

A Visiting Nurse's day isn't spent performing the same task over and over.

One patient may need complex wound care.

Another may need medication education following a hospitalization.

Another may require a comprehensive assessment because something has changed.

Another family may simply need an experienced RN who can answer questions, recognize what's happening clinically, and help them understand what comes next.

Every door you walk through brings a different patient and a different clinical challenge.

For nurses who enjoy variety and using their assessment skills, that's one of the best parts of home health.

What You'll Do

Provide skilled nursing visits throughout the Northeast territory.
Perform comprehensive nursing assessments.
Provide wound care, medication management, disease education, and other skilled interventions.
Monitor patients for changes in condition and communicate concerns appropriately.
Educate patients and caregivers on medications, safety, disease management, and their plan of care.
Assist with admissions and additional visits as patient needs require.
Support surrounding territories based on census and staffing needs.
Collaborate with RN Case Managers, physicians, therapists, social workers, and other interdisciplinary professionals.
Complete accurate and timely clinical documentation.
Complete OASIS documentation when applicable.
Participate in an on-call rotation.
Participate in a weekend rotation.

The Best Visiting Nurses Can Walk Into a Home and Take It From There

You won't have another nurse standing beside you during every visit.

That's why we're looking for an RN who is comfortable being trusted.

You can assess what's happening.

You recognize when something has changed.

You know when to educate, when to intervene, and when something needs to be escalated.

At the same time, independent doesn't mean unsupported. You'll have Case Managers, clinical leadership, and an interdisciplinary team available to collaborate when you need them.

Who We're Looking For

Active Minnesota Registered Nurse license required
Strong clinical assessment and critical-thinking skills
Confidence providing hands-on skilled nursing care
Ability to work independently in patient homes
Excellent patient and caregiver education skills
Strong organization and time-management abilities
Valid driver's license and reliable transportation
Willingness to participate in on-call and weekend rotations
Home health experience preferred
OASIS experience preferred
