Hospice RN Case Manager (Part-Time)

TREK Healthcare

We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our hospice team. This role is ideal for an RN who values meaningful patient relationships, interdisciplinary collaboration, and providing end-of-life care with dignity, empathy, and excellence.

Position Details

Schedule: 30 hours per week working Monday, Tuesday, Thursday and Friday, with Wednesday off. Daily work hours may vary based on patient and business needs.
On-Call Requirements:
One on-call weekend per month
Two on-call nights per month
Pay: $34.00–$54.25 per hour, depending on years of experience and qualifications
Employment Type: Part-Time

Key Responsibilities

Provide direct nursing care to hospice patients in their place of residence
Serve as the primary case manager, overseeing and coordinating all aspects of patient care
Manage and collaborate with interdisciplinary team members, including: LPNs, Hospice aides (bath aides), Social workers, chaplains, and other disciplines
Develop, implement, review, and update individualized plans of care
Complete accurate, timely clinical documentation and charting
Follow up on care plans, including reassessments and resubmissions as required
Communicate effectively with patients, families, physicians, and other healthcare professionals
Provide education and emotional support to patients and families, leading with empathy and compassion
Participate in on-call rotation to support patients after hours as needed
Ensure compliance with hospice regulations, agency policies, and standards of care

Qualifications

Current Registered Nurse (RN) MT license
Hospice or home health experience preferred (but not required for the right candidate)
Strong clinical assessment and care coordination skills
Excellent communication, organization, and documentation abilities
Ability to work independently while collaborating with a multidisciplinary team
Valid driver’s license, proof of insurance coverage, and reliable transportation
EMR experience preferred
