Full Job Description

Carolina Hearts Home Care is seeking a dedicated and experienced Registered Nurse (RN) to join our team. The RN Care Manager will play a crucial role in enhancing the quality of care provided to our clients by overseeing and developing our caregiving staff. This position requires strong communication skills, a commitment to client care, and the ability to work collaboratively with team members and supervisors.

Responsibilities:

Improve overall care provided to clients by Carolina Heart’s caregivers and staff.
Focus on basic client care monitoring and developing a knowledgeable and professional caregiver staff.
Conduct assessments and reassessments of clients for in-home aide services.
Supervise in-home aides and client care.
Assist the Nursing Supervisor with agency Quality Assurance (QA) activities, including every 90-day client record reviews.

Education/Qualifications:

Current, valid Registered Nurse (RN) License with the NC Board of Nursing.
Must complete and pass a criminal background check.
Must complete the health screening, testing, and immunization requirements of the agency.
Must have work status verified by E-verify per agency personnel policy.
Must not be listed on the North Carolina Medicaid Provider Termination and Exclusion list or the US DHHS Office of Inspector General (OIG) list of excluded individuals/entities upon hire or during monthly checks.

Registered Nurse Assessment and Development of the In-Home Aide Plan of Care:

Follow DHSR 10 A NCAC 13J rules regarding client assessment and plan of care for in-home aide services.
Ensure the plan of care contains the level of assistance required by the client for each ADL.

Registered Nurse Supervision of In-Home Aides:

Follow NC DHSR Home Care licensure rules and client payer source requirements for supervision frequency.
Document supervision visits and ensure in-home aides demonstrate competence.
Conduct supervisory visits every 90 days and annually, including a review of the client’s condition and response to services.
Maintain documentation of supervisory visits and corrective actions as needed.

Caregiver Mentor:

Serve as a caregiver mentor to improve overall care by creating a well-trained, professional, and knowledgeable caregiving staff.
Orient new caregivers to agency policies and procedures.
Assist with new hire skills validation.
Mentor new hires through their first 90 days with the agency.
Conduct hands-on training with caregivers as needed.

Pay: $35.00 - $40.00 per hour

Expected hours: 36.0 – 40.0 per week
