About the Role:

As a Case Manager at Children's Advocate LLC, you will play a vital role in supporting children and families in need. Join our dedicated team in Frankfort, IL, and make a meaningful impact by guiding clients through challenging situations with compassion and expertise.

Responsibilities:

Conduct comprehensive assessments to identify client needs and develop tailored service plans.
Provide ongoing support and advocacy for children and families in various situations.
Collaborate with community resources and service providers to ensure holistic care.
Maintain accurate case documentation and track progress towards goals.
Facilitate family meetings and coordinate services among multiple stakeholders.
Monitor client progress and adjust service plans as necessary.
Educate families about available resources and support systems.
Participate in training and professional development opportunities to enhance skills.

Requirements:

Bachelor’s degree in Social Work, Psychology, or a related field.
Minimum of 2 years of experience in case management or social services.
Strong communication and interpersonal skills for effective client interaction.
Knowledge of child welfare laws and community resources.
Ability to work independently and as part of a multidisciplinary team.
Compassionate demeanor with a commitment to advocating for children and families.
Valid driver's license and reliable transportation for client visits.
