Full Job Description

Job Overview
We are a Home Health Agency is seeking a dedicated and organized Benefits & Community Relations Coordinator to support clients in navigating healthcare benefits while building strong relationships with community partners and referral sources.

This position is primarily field-based and is ideal for someone with experience in healthcare benefits, insurance, case management, social services, or benefits coordination. The successful candidate will assist clients with benefit-related needs, support IDoA and DORS program requirements, and serve as a liaison between clients, community organizations, and the agency.

Key Responsibilities

Client & Benefits Support

Meet with clients to assist with healthcare benefit-related needs.
Support IDoA and DORS program requirements, eligibility, and documentation.
Educate clients on available programs and services.
Maintain accurate client records and complete timely follow-ups.
Address questions or concerns related to benefits access and eligibility

Community Relations

Build and maintain relationships with referral sources and community partners.
Represent the agency professionally during community visits and meetings.
Assist in developing referral opportunities and strengthening community partnerships.

Administrative Responsibilities

Update referral tracking systems and reports.
Maintain organized documentation and client records.
Provide regular updates and reports to the Administrator.
Assist with other administrative tasks as assigned.

Compliance & Confidentiality

Ensure compliance with HIPAA and all applicable state and federal regulations
Maintain strict confidentiality of client and employee information
Support audits, reporting, and documentation requests related to benefits and eligibility

Qualifications

Valid driver's license with a good driving record.
Ability to travel locally for client visits and community meetings.
Ability to pass a background check.
Strong communication, interpersonal, and organizational skills.
Ability to manage multiple priorities and work independently.

Preferred Experience

Candidates with experience in any of the following are strongly encouraged to apply:

Strong documentation and follow-up skills.
Bilingual (English/Spanish) preferred but not required.
