Care Coordinator – Job Description

Chico Housing Action Team (CHAT)
Reports To: Social Services Program Manager
Classification: Non-Exempt, Hourly

Job Summary

The Care Coordinator supports CHAT residents and Medi-Cal beneficiaries in accessing and maintaining Community Supports (CS) services and coordinated care across physical, behavioral, dental, developmental, and social service systems. This role ensures compliance with CalAIM guidelines, ECM protocols, and billing requirements by tracking outcomes, supporting resident health behavior changes, and collaborating with providers and partners. Care Coordinators help individuals and families stabilize and sustain housing through structured care coordination, linkage to community resources, and long-term tenancy retention support.

Essential Duties

Care Coordination & Case Management

· Serve as the primary point of contact for assigned residents, including high-acuity Medi-Cal beneficiaries.

· Develop, update, and monitor Individual Health Action Plans aligned with CalAIM requirements.

· Provide ECM services using motivational interviewing and trauma-informed, strength-based practices.

· Coordinate healthcare, behavioral health, and supportive services to ensure comprehensive care.

· Conduct basic health screenings (e.g., PHQ-9) as required by ECM protocols.

· Educate residents on self-management skills and empowerment strategies.

· Participate in intake, onboarding, and eligibility assessments to ensure appropriate service alignment.

Housing Stability & Community Supports

· Assist residents in maintaining housing and addressing emerging barriers to stability.

· Identify early warning signs of housing instability and perform eviction-prevention interventions.

· Conduct regular housing-focused check-ins and tenancy-support services.

· Collaborate with landlords, property managers, and community housing partners.

· Educate residents on tenant rights and responsibilities.

· Assist residents in connecting to benefits, employment, education, and other stabilizing supports.

Documentation, Compliance & Reporting

· Document all services in HMIS and other required systems within 3 business days.

· Ensure documentation meets DHCS, ECM, and MCP billing and regulatory requirements.

· Track outcomes, monitor resident progress, and contribute to continuous quality improvement.

· Maintain confidentiality and professional standards in all interactions.

· Perform fieldwork in homes, institutions, and community settings as needed.

· Participate in required training and ongoing competency development related to CalAIM compliance.

Qualifications

Valid driver’s license and background clearance required.
Experience working with diverse or vulnerable populations.
Strong communication, organization, and documentation skills.
Ability to work independently and collaboratively within a team environment.
Ability to consistently meet documentation and service delivery timelines.

Skills & Abilities

Case management experience preferred.
Knowledge of housing processes, tenancy supports, and local resources.
Ability to de-escalate crises and support individuals with complex needs.
Proficiency in Microsoft Office and database systems.
Strong problem-solving, adaptability, and time-management abilities.

Education & Experience

Bachelor’s degree in Social Work, Public Health, Human Services, or related field preferred.
Lived experience or professional experience supporting individuals experiencing homelessness or behavioral health needs strongly valued.

Pay: $22.00 - $25.00 per hour
