Emergency Response Declaration:
FHC is a provider of public health services. FHC and its staff maintains an ethical responsibility to remain ready and available to serve in the event of a Natural Disaster or during a declaration for a State of Emergency. FHC and its staff must be prepared to support the local government and/or the Virgin Islands community with access to health care. Description Summary:
In this fast-paced environment, The Behavioral Health Case Manager will provide comprehensive case management services to individuals with mental, emotional, or substance abuse disorders in an outpatient setting. The position will coordinate care, track progress and ensure clients receive access to necessary services and resources to support their overall health and well-being. The Case Manger will develop and implement treatment plans and monitor progress in collaboration with healthcare providers, community organizations and other support systems to ensure clients receive holistic, patient centered care. The position will be required to provide ongoing support, advocacy and education and serve as a point of contact for clients and their families.
Essential Functions:

Assess clients to determine needs in the areas of behavioral health, mental health, substance use or social services and develop individualized treatment plans to address short and long term goals.

Coordinate, facilitate and advocate for clients to ensure they access required services and resources such as behavioral health services, medical care, housing employment and other community resources.

Establish good working relationships and exert influence with support networks, government resources, and community resources to assist and provide support and resources for clients

Organize and influence community and other agencies to develop programs, policies, and services to assist in social work

Serve as the primary point of contact for clients and their families and ensure that services are timely, appropriate, and aligned with patient-centered treatment plans.

Help manage crises by developing crisis management plans, providing support during crisis and helping clients develop coping mechanisms

Serve as a key member of an interdisciplinary team of physicians, nurses, social workers, therapists, and other providers, to ensure a coordinated approach to care.

Participate in case conferences and share relevant information to ensure effective client care and outcomes.

Create programs and services to benefit clients and community

Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and follow-up reports, in compliance with agency policies and regulatory requirements.

Ensure documentation is completed timely and in accordance with confidentially rules and HIPAA compliance

Position requirements / Job qualifications:
Education and Training: Bachelor’s degree in social work, psychology, sociology, healthcare or medical and related fields; Community Health Worker Certificate a plus; Master’s degree in social work preferred; supervised practicum or internship preferred. Current CPR/BLS certificate.
