Job Overview

Location: Columbus, Ohio (Primary Office)
Position Type: Full-Time (40 Hours per Week)
Compensation: $17.00–$20.00 per Hour (Based on Experience)

Position Summary

Solace at Vivian's is seeking a compassionate, dependable, and motivated Behavioral Health Case Manager to provide comprehensive case management and care coordination services to adults and adolescents receiving behavioral health and substance use treatment. The Case Manager works closely with the multidisciplinary treatment team to assist clients in accessing community resources, achieving treatment goals, maintaining housing stability, improving overall functioning, and promoting long-term recovery.

This position requires excellent communication, organization, documentation, and problem-solving skills. The ideal candidate is comfortable working with individuals experiencing mental illness, substance use disorders, homelessness, and justice involvement.

Minimum Qualifications

High School Diploma or GED required.
Current Care Coordinator and/or Case Management Certification required.
Active Ohio Medicaid Billing Number required.
Valid Ohio Driver's License.
Reliable transportation.
Current automobile insurance.
Ability to pass state and federal background checks.
Basic computer proficiency and ability to document services in an Electronic Health Record (EHR).

Preferred Qualifications

One year of experience working in behavioral health, substance use treatment, community mental health, or social services.
Experience working with justice-involved individuals, including probation and parole populations.
Knowledge of Ohio Medicaid documentation and billing requirements.
Familiarity with community resources throughout Central Ohio.

Essential Job Duties

Provide community-based behavioral health case management services.
Coordinate behavioral health, medical, psychiatric, housing, employment, educational, and social service needs.
Complete assessments and participate in treatment planning with the interdisciplinary treatment team.
Assist clients in obtaining Medicaid, SNAP benefits, SSI/SSDI, identification documents, employment, housing, transportation, and community resources.
Conduct office visits, home visits, community visits, hospital visits, and telehealth appointments as appropriate.
Monitor client progress toward individualized treatment goals.
Maintain regular communication with psychiatrists, therapists, nurses, primary care providers, hospitals, probation/parole officers, and other referral sources.
Participate in crisis intervention and safety planning when clinically appropriate.
Advocate for clients while promoting independence and recovery.
Assist with discharge planning and continuity of care.
Participate in treatment team meetings, supervision, and required trainings.
Maintain professional boundaries and uphold agency policies at all times.
Perform other duties as assigned.

Documentation Responsibilities

Complete all Medicaid-compliant documentation accurately and within agency deadlines.
Maintain timely progress notes for all billable services.
Maintain accurate electronic health records.
Meet established productivity standards.
Ensure documentation complies with Medicaid, HIPAA, Joint Commission, and Solace at Vivian's policies and procedures.

Travel Requirements

Primary work location: Columbus, Ohio.
Occasional travel to the Dayton and Cincinnati office locations may be required approximately once per month for staff meetings, trainings, client services, or operational needs.
Employees may utilize a company van for approved travel between Solace at Vivian's office locations, subject to availability and agency policies.
Employees must maintain a valid Ohio driver's license and an acceptable driving record to operate company vehicles.

Physical Requirements

Ability to sit, stand, walk, bend, and lift up to 25 pounds.
Ability to travel throughout the community to provide services.
Ability to use computers, tablets, and mobile devices for documentation and communication.

Work Environment

Services are provided in office settings, clients' homes, hospitals, correctional facilities, schools, community agencies, and other approved community locations. Employees work closely with individuals experiencing serious mental illness, substance use disorders, homelessness, and criminal justice involvement. Flexibility, professionalism, and the ability to adapt to changing client needs are essential.
