Full Job Description

Job Summary

We are seeking a highly skilled and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will possess extensive knowledge of the Minimum Data Set (MDS) process, Medicare and Medicaid regulations, and clinical documentation standards. As an MDS Coordinator, you will be responsible for ensuring accurate and timely completion of MDS assessments, optimizing reimbursement, and maintaining compliance with federal and state healthcare regulations. Your expertise in clinical documentation, coding, and utilization management will support high-quality patient care and operational efficiency across long-term care, skilled nursing facilities, or inpatient settings.

Essential Duties and Responsibilities:

· Coordinate and manage the timely completion of the MDS assessments, Care Area Assessments (CAAs), and care plans in accordance with CMS guidelines.

· Ensure accuracy of coding within the MDS to reflect resident care, services, and needs.

· Collaborate with interdisciplinary team members to gather assessment data and input for comprehensive resident assessments and care planning.

· Monitor MDS schedules to ensure completion within required timeframes.

· Submit MDS assessments to CMS and ensure acceptance; correct and resubmit any rejected or erroneous files.

· Track Quality Measures (QMs), Quality Indicators (QIs), and Resident Assessment Validation and Entry (RAVEN) system reports.

· Educate staff on RAI process and MDS accuracy; provide training as needed.

· Ensure care plans are individualized, updated regularly, and reflect the resident’s condition and needs.

· Participate in Quality Assurance and Performance Improvement (QAPI) programs.

· Monitor documentation to ensure compliance and support for MDS assessments and reimbursement.

· Lead weekly IDT meeting.

· Stay current with changes in regulations, reimbursement practices, and best practices for MDS management.

Qualifications:

Education and Experience:

· Licensed Registered Nurse (RN)

· Minimum of 2 years of experience in long-term care or skilled nursing facility.

· Prior experience with MDS 3.0 and RAI process required.

· RAC-CT (Resident Assessment Coordinator – Certified) certification preferred.

Knowledge, Skills, and Abilities:

· Strong knowledge of Medicare and Medicaid reimbursement systems.

· Proficiency with MDS software (e.g., PointClickCare, MatrixCare, etc.).

· Excellent organizational, analytical, and communication skills.

· Ability to collaborate with interdisciplinary teams and prioritize multiple tasks.

· Detail-oriented with strong data management and assessment skills.

Working Conditions:

· Work is primarily performed in a healthcare facility with moderate noise levels.

· May require sitting for extended periods and use of computer equipment.

· Occasional evening or weekend work may be required.
