Full Job Description

Chronic Care Coordinator

Roles and Responsibilities

Education:

Educate and aid individuals and communities in accessing and using the health and social services needed.
Implementation and development of outreach strategies as needed.
Develop, plan, and implement an educational curriculum classes one-on-one, in-person or telehealth (virtual) modality using a variety of education methods and materials, such as: decision-making, behavioral modification, and motivational interviewing process for individuals with chronic diseases.
Implement and evaluate comprehensive education programs, by: collecting, reviewing, and inputting patients’ data into electronic systems, and attending routinely CQI meetings with medical providers.
Monitoring of medical screenings such as blood pressure, BMI, waist circumference, blood glucose, Cholesterol, and A1C.
Conduct education in a manner that matches the linguistic and cultural needs of the participants and community. Build health literacy and cross-cultural communication.

Care Coordination:

Keep up to date knowledge of all Project Vida services offered and community social services as well as the proper referral processes.
Conduct an initial assessment of all patients that meet the criteria for the chronic disease care coordination program to determine their health needs.
Create and promote adherence to a care and medication and treatment plan developed in coordination with the patient, primary care provider, and family/caregiver(s), and showcases the patient’s treatment options and preferences.

Electronic Health Records/CCM and PCM:

Advanced proficiency in navigating and utilizing ECW (eClinicalWorks) systems
Strong technical aptitude with the ability to quickly adapt to new technologies and platforms
Demonstrated experience working with Electronic Health Records (EHR) and similar data management systems
Ability to effectively align and integrate workflows with client enrollment processes within ECW
Proficient in managing and executing client intake, registration, and enrollment within the ECW platform
Strong understanding of workflow optimization to ensure efficient, accurate data entry and streamlined client processing
Knowledge of Care Coordination Management (CCM) and Principal Care Management (PCM) workflows within an EHR environment
Ability to collaborate effectively within a multidisciplinary care team to support optimal patient outcomes, well-being, and overall success
Other duties assigned by the supervisor.

Education Requirements

Community Health Worker Certification by the state of Texas
At least one year of work experience in this or similar field
Bilingual (English/Spanish)
Culturally competent in border culture to provide culturally appropriate education and information to a broad range of audiences.
Computer, internet and analytical skills are required. (e.g. technical proficiency in GoogleDocs, Microsoft PowerPoint, Excel, and Word)

Preferred Skills

Strong advocacy, education, and facilitation skills
Knowledge of chronic conditions, their management and prevention (e.g. diabetes, hypertension, pre-diabetes)
Strong organizational and professional skills (e.g. attention to detail, balance priorities and time management)
Must be self-directed and require limited oversight in performance of duties.
Must possess strong interpersonal and relationship building skills.
Strong written and verbal communication, listening and public speaking skills

Additional Requirements

Must have a reliable vehicle with current liability insurance and a valid driver’s license.
Ability to lift and carry items up to 25 lbs.
Ability to observe ethical and legal standards (e.g. HIPAA)
Must be able to work non-traditional hours and weekends on occasions.
