DMV Area · Full-Time · Field-Based

About this role

We are a home care company serving the DMV that provides all levels of care, from companion and personal care services to skilled nursing. This role is focused specifically on growing the skilled nursing side of our business. We are looking to build referral relationships with hospitals, rehab facilities, physician offices, and other providers throughout the region who work with patients that need a higher level of care at home.

The position starts in Virginia and expands into Maryland and DC. We are looking for someone who is comfortable in the field, understands the clinical side of what we do, and can represent our services credibly to the people who make referral decisions.

What you'll be doing

· Visit hospitals, rehab facilities, physician offices, wound care clinics, and other healthcare providers across the DMV to introduce our services and build referral relationships

Connect with discharge planners, case managers, social workers, and clinical staff who work with patients transitioning home from a higher level of care
Represent a broad range of skilled nursing services including infusion therapy, wound care, medication management, and care for medically complex patients
Attend community events and health fairs throughout the region
Track your activity and report on progress to leadership
Work closely with our clinical and operations team to make sure new referrals have a smooth start

Who we are looking for

Home care experience is a plus but not required. What matters most is that you understand skilled clinical care and can have a real conversation with a discharge planner or case manager about what we offer and why it matters. The DMV healthcare community is relationship-driven, and we need someone who knows how to show up consistently and earn trust over time. Strong candidates will come from one of these backgrounds:

· A licensed nurse (LPN or RN) interested in moving into business development

Someone with skilled intake experience at a SNF or hospital
A healthcare marketer with exposure to skilled or specialty services

You should be ambitious, comfortable being out in the field, good at following up, and genuinely interested in building something.
