PT, OT & SLP – Certified Home Health Agency (CHHA)

Service Areas: New York City – Five Boroughs (Brooklyn, Bronx, Queens, Manhattan & Staten Island), Nassau County, and Westchester County
Setting: Home Health / Field-Based
Employment: Full-Time, Part-Time and Per-Diem Opportunities

Position Summary

We are seeking compassionate and experienced Physical Therapists (PT), Occupational Therapists (OT), and Speech-Language Pathologists (SLP/ST) to provide skilled rehabilitation services to patients in their homes throughout the five boroughs of New York City, Nassau County, and Westchester County.

Therapists will work as part of an interdisciplinary home health team to evaluate patients, develop individualized plans of care, provide skilled treatment, educate patients and caregivers, and help patients achieve the highest possible level of safety, independence, and functional ability in the home.

Physical Therapist (PT) Responsibilities

Perform comprehensive physical therapy evaluations and reassessments in the patient's home.
Develop and implement individualized treatment plans based on physician/provider orders and patient goals.
Provide skilled interventions addressing strength, balance, endurance, mobility, transfers, gait, and functional limitations.
Provide gait training and appropriate instruction in the use of assistive devices.
Evaluate home safety and recommend appropriate durable medical equipment (DME) and adaptive equipment.
Develop and instruct patients and caregivers in individualized home exercise programs.
Monitor patient progress and modify treatment plans as clinically indicated.
Educate patients and caregivers regarding fall prevention, mobility, transfer techniques, and safety.
Communicate changes in patient condition to the physician/provider, clinical supervisor, RN case manager, and other members of the interdisciplinary team.
Complete all required clinical documentation accurately and within agency-required timeframes.
Participate in care coordination and discharge planning.

Occupational Therapist (OT) Responsibilities

Perform comprehensive occupational therapy evaluations and reassessments in the patient's home.
Assess the patient's ability to safely perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Develop individualized treatment plans to improve independence and safety.
Provide therapeutic interventions related to dressing, bathing, grooming, toileting, feeding, meal preparation, and other functional activities.
Provide upper-extremity strengthening, coordination, range-of-motion, and functional training as appropriate.
Assess the home environment and recommend adaptive equipment, DME, and environmental modifications.
Educate patients and caregivers regarding energy conservation, joint protection, fall prevention, and safe performance of daily activities.
Establish and teach individualized home exercise and activity programs.
Monitor progress and modify the treatment plan based on the patient's clinical and functional status.
Coordinate care with physicians/providers, nurses, PTs, SLPs, social workers, home health aides, and other members of the interdisciplinary team.
Complete required clinical documentation accurately and timely.
Participate in discharge planning and development of appropriate long-term safety strategies.

Speech-Language Pathologist (SLP/ST) Responsibilities

Perform comprehensive speech-language pathology evaluations and reassessments in the patient's home.
Evaluate and treat speech, language, communication, cognitive-communication, and swallowing disorders within the therapist's scope of practice.
Develop individualized plans of care based on assessment findings and patient goals.
Provide skilled therapy for dysphagia and swallowing difficulties as clinically appropriate.
Provide interventions for speech, language, voice, cognition, memory, and communication deficits.
Develop strategies to improve patient safety and functional communication within the home.
Educate patients, family members, and caregivers regarding swallowing precautions, communication strategies, and therapeutic exercises.
Monitor patient response to treatment and modify interventions as appropriate.
Communicate significant findings and changes in condition to the physician/provider and interdisciplinary care team.
Coordinate services with nursing, PT, OT, social work, home health aides, and other disciplines involved in the patient's care.
Complete all clinical documentation accurately and within required agency timeframes.
Participate in care coordination and discharge planning.

General CHHA Responsibilities

All therapists are expected to:

Provide skilled services in accordance with the established plan of care and applicable professional standards.
Maintain effective communication with the patient's physician/provider, RN case manager, clinical management, and interdisciplinary team.
Identify and promptly report changes in the patient's condition or safety concerns.
Provide patient and caregiver education appropriate to the patient's needs and plan of care.
Maintain accurate, complete, and timely documentation of evaluations, visits, reassessments, progress, care coordination, and discharge.
Participate in interdisciplinary care coordination as required.
Maintain patient confidentiality and comply with HIPAA requirements.
Follow agency policies and procedures, infection-control practices, and applicable federal and New York State requirements.
Maintain a professional and compassionate approach while providing services in patients' homes.
Manage an assigned geographic territory and visit schedule efficiently.

QualificationsPhysical Therapist

Current and unrestricted New York State Physical Therapist license.
Graduate of an accredited Physical Therapy program.
Home health experience preferred.
Strong clinical assessment, communication, and documentation skills.

Occupational Therapist

Current and unrestricted New York State Occupational Therapist license.
Graduate of an accredited Occupational Therapy program.
Home health experience preferred.
Strong clinical assessment, communication, and documentation skills.

Speech-Language Pathologist

Current and unrestricted New York State Speech-Language Pathologist license.
Graduate degree in Speech-Language Pathology from an accredited program.
Home health and/or dysphagia experience preferred.
Strong clinical assessment, communication, and documentation skills.

Additional Requirements

Ability to travel to patients' homes within the assigned service territory.
Reliable transportation appropriate for the assigned geographic area.
Ability to work independently in a field-based environment while collaborating with the interdisciplinary team.
Excellent time-management and organizational skills.
Ability to meet agency documentation and productivity requirements.
Current required professional credentials and certifications in accordance with agency policy and applicable regulations.
