Full Job Description

Job Overview
We are a service provider agency serving the Inland Empire, and delivering high-quality Coordinated Family Support (CFS) Services to individuals with intellectual and developmental disabilities (IDD). Our agency is committed to person-centered practices, cultural responsiveness, and empowering families through meaningful community connections and long-term support planning.

We are seeking compassionate, organized, and self-motivated CFS Caseworkers who are passionate about supporting individuals and families in navigating services and achieving greater stability and independence.

The Coordinated Family Support (CFS) Caseworker provides strengths-based case management services to families served by Inland Regional Center. The CFS Caseworker assists families in identifying needs, coordinating community resources, advocating for services, and promoting stability within the home and community.

This role requires travel within the Inland Empire region.

Duties

Conduct intake meetings and ongoing needs assessments
Develop individualized service plans in collaboration with families
Coordinate and monitor community resources (housing, medical, IHSS, SSI, food assistance, mental health, etc.)
Maintain regular contact with families through home visits, phone, and virtual meetings
Collaborate with agency management team and other providers
Provide advocacy and support to ensure families access appropriate services
Maintain timely, accurate, and compliant documentation
Submit quarterly progress reports and daily service notes
Participate in team meetings and required trainings

Strong interpersonal and communication skills
Cultural humility and family-centered approach
Professional boundaries and ethical practice
Crisis response and problem-solving abilities
Ability to manage a caseload of 3-5 consumers and meet deadlines
Strong expertise in crisis intervention and crisis management techniques
Experience in social work or related fields with supervising or mentoring responsibilities preferred
Knowledge of Welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives
Excellent communication skills with the ability to motivate clients through motivational interviewing methods
Ability to handle sensitive situations with empathy, professionalism, and transparency
Demonstrated organizational skills for managing multiple cases efficiently while maintaining detailed documentation

Qualifications

Required:

Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or related field
Minimum 1 year experience working with individuals with developmental disabilities
Strong knowledge of community resources
Excellent documentation and organizational skills
Reliable transportation, valid driver’s license, and auto insurance
Ability to work independently in a community-based setting
Must be Spanish Speaking

Preferred:

Experience working with Regional Center consumers
Spanish Speaking
Knowledge of SSI, IHSS, Medi-Cal, and public benefit systems
