Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing personalized care plans, and connecting them with necessary resources. This position requires strong interpersonal skills and the ability to work collaboratively with various stakeholders to ensure the well-being of clients.

Responsibilities

Conduct comprehensive assessments to identify client needs and develop tailored service plans.
Provide crisis intervention and management to clients in urgent situations.
Offer addiction counseling and support for individuals struggling with substance use disorders.
Collaborate with healthcare providers, social workers, and community organizations to coordinate care.
Monitor client progress and adjust service plans as necessary to achieve desired outcomes.
Facilitate access to behavioral health services, childcare resources, and workforce development programs.
Mentor clients in developing life skills and coping strategies for improved self-sufficiency.
Maintain accurate documentation of client interactions, progress notes, and case files in compliance with regulations.
Supervise interns or junior staff members as needed, providing guidance and support in case management practices.

Skills

Proficiency in crisis intervention techniques and crisis management strategies.
Strong understanding of behavioral health issues and available resources.
Familiarity with motivational interviewing techniques to engage clients effectively.
Supervising experience is a plus for overseeing team members or interns.
Knowledge of Medicare processes related to client care is advantageous. This role requires a proactive approach to problem-solving, excellent communication skills, and a genuine desire to make a positive impact on the lives of those we serve.
