SUMMARY OF RESPONSIBILITIES:

Will function in support of Ml and/or I/DD Clinical Support Services. Primary areas of responsibility include completed nursing assessments, care plans, monitoring the individual's health care needs, administering injections, and obtaining vital signs. Providing services in a variety of settings to include the outpatient clinic, community, individual's home, or an AFC setting. Will be required to complete Initial/Annual Care Plan and Nursing assessments, based on need. Writes appropriate health care programs focusing on integrated health needs, reviewing labs and treatments as ordered by physicians; consultation and collaboration with providers and staff; monitors progress and case closures as assigned. Address daily medication calls and consults with Physicians, Physician Assistant, and Nurse Practitioner. Discharge of individual services for Behavioral Health Home. Meets regularly with the care team to plan care and discuss cases, and exchanges appropriate information with team members. Participates in crisis intervention backup services along with after-hours on-call as needed if a Registered Nurse.

QUALIFICATIONS:

Education: Registered Nurse Degree or License Practical Nurse from a recognized or accredited institution with coursework in mental health.

Experience: Preferred one or more years of relevant mental health experience. Good communication skills (both verbal and written) are required. Possess State registration as a Registered Nurse or

Licensed Practical Nurse. Functional computer literacy is required within six months from date of

hire. Lived experience with mental illness, intellectual and developmental disabilities, and/or

substance use disorders valued.

ESSENTIAL JOB FUNCTIONS:

1. Demonstrates knowledge of universal precautions, standards of practice including but not limited to

confidentiality and recipient rights, and emergency protocol.

2. Participates in the selection of strategies to implement evidence-based wellness and prevention initiatives, and initial care plan develop including goals.

3. Demonstrate effective health care assessments and written health care plans/ guidelines that adhere to "Person-Centered Planning".

4. Demonstrates effective case recording in accordance with agency procedures.

5. Demonstrate knowledge of policies and procedures for dispensing medications in a residential Adult Foster Care (AFC) or personal home.

6. Provides education on medical diagnoses and support along with family support as necessary.

7. Collaborate with other staff in diagnosing problems and evaluating the individual served.

8. Work must comply with guidelines established by PIHP, MDHHS, Medicaid and any other regulatory/monitoring agencies that protect individuals served.

9. Communicates with medical providers, subspecialty providers, including mental health and substance use service providers, long-term care, and hospitals regarding records, including admission and discharge.

10. Will demonstrate functional computer literacy in all agency documentation.

11. Will be required to maintain highest degree of licensure or state registration specific to their position.

12. Staff will be expected to participate in trainings aimed at the provision of services for individuals served with co-occurring disorders.

13. Provides in-service training and consultation to staff, individuals served, their families, agencies, and other individuals, as required.

NON-ESSENTIAL JOB FUNCTIONS:

1. Performs other related duties as required.

2. May be requested to be involved in the Board's continuous quality improvement program.

3. May travel to offices and other agency sites in the agency catchment area.

4. May be required to provide own transportation while performing agency-related duties.

5. May be required to participate in the 24-hour emergency services through agency Emergency Services.

6. Each fiscal year will be required to complete 24 hours of continuing education (CE) training specific to children and in accord with Policy 5135 if a Registered Nurse.

Ability to walk, bend, stand, sit, lift up to 25 lbs., with or without assistance, stretch/reach, hear, see, hand/finger dexterity, and drive.
