Full Job Description

Company Overview

UMCF is a comprehensive mental health and child welfare agency dedicated to helping families stay together, reunify, or establish new permanent families. We serve thousands of children and parents across Ohio through various programs, including Foster Care, Outpatient Mental Health Services, Psychiatry, Intensive Home-Based Treatment, Bridges, Independent Living, Care Coordination, and Adoption.

Summary

Provide supportive services for clients who have emancipated from the foster care system.

Responsibilities

Duties and Responsibilities

1. Case management of Bridges enrollees.

2. Appear in court with the youth 180 days after enrollment and annually thereafter.

3. Ensure contact is attempted with potential Bridges candidate within twenty-four (24) hours from receipt of referral.

4. Assess referrals and make determinations regarding enrollment or referral close-out within 30 days.

5. Complete documentation for monthly financial requests as outlined in the financial agreement and complete documentation required in SACWIS within 10 business days per ODJFS.

6. Identify appropriate placement for enrollee, set up housing, utilities, and furniture for clients who require apartment placements.

7. Collect eligibility documentation from enrollees monthly to verify program eligibility.

8. Involve support or biological family members in as many decisions as possible about individual goals to assist the youth after Bridges program participation ends.

9. Provide face to face as well as phone contact with youth as required by the Tier Identification tool. Document these activity logs and contacts in the SACWIS system within 5 business days.

10. Monitor cases monthly for case closure and provide enrollees with necessary documents.

11. Provide crisis management – on call 24/7/365.

12. Attend staff meetings (once a month – dept and all staff) and two meetings a month.

13. Attend Collaborative Bridges meeting quarterly.

14. Schedule and attend supervision (frequency varies, minimum of one time per month).

15. Attend all mandatory training required to satisfy company and licensure requirements (CEUs, Agency-specific, COA required or required by clinical department).

16. Support new team members with job shadowing opportunities.

17. Demonstrate an understanding and cultural sensitivity for diverse populations.

Qualifications An example of acceptable qualifications (at least one of the following):

Must possess a bachelor’s degree in social work or a related field.

Or any equivalent combination of education, experience, and training which provides the required knowledge, skills, and abilities.

Knowledge, Skills, and Abilities

Knowledge in crisis intervention, child and adolescent development, and community services.
Ability to demonstrate clinical skills and capacity to build relationships with clients served.
Ability to understand the speech of another person.
Ability to speak clearly so listeners can understand.
Ability to see details of objects that are less than a few feet away.
Ability to maintain focus on one source of sound.
Ability to drive in a variety of weather and light conditions.
Ability to administer private, nonprofit service organizations, including an understanding of the legal responsibilities and constraints of such organizations.
Ability to obtain and manage external grants and contracts.
Ability to communicate effectively.
Ability to establish and maintain effective working relationships with federal, state, regional, and local agencies, community leaders, and the general public.
Ability to express oneself clearly and effectively in oral and written form.
