Overview

Join our dynamic healthcare team as a Case Manager in Lubbock, where your expertise will make a meaningful difference in patient outcomes and community health. In this vital role, you will coordinate comprehensive care plans, facilitate seamless discharge processes, and connect patients with essential community resources. Your proactive approach and compassionate communication will empower individuals to navigate complex health journeys confidently. Responsibilities

Conduct thorough patient assessments, including medical history review, physiological evaluation, and mental health screening to determine individualized care needs.
Develop and implement tailored case management plans that encompass inpatient, outpatient, post-acute, and community-based services.
Coordinate discharge planning by collaborating with healthcare teams, patients, families, and community resources to ensure safe transitions of care.
Facilitate diagnostic evaluations and behavioral health assessments for diverse populations, including children and individuals with disabilities or substance use disorders.
Provide patient service through active communication, education on medical terminology, and support for navigating healthcare systems.

Experience

Proven hospital experience with a strong background in acute care settings such as mental health hospital and group homes.
Experience working with adults with severe mental health conditions, or those with substance use disorders.
Familiarity with managed care processes, utilization management, and discharge planning across inpatient and outpatient environments.
Proficiency in EMR/EHR systems, medical records management, and medical terminology essential for accurate documentation.
Demonstrated ability to work effectively with diverse cultural backgrounds and coordinate community resources for comprehensive patient support.
Strong skills in client communication, social work principles, task prioritization, and working collaboratively with multidisciplinary teams.
Relevant certifications such as Basic Life Support (BLS), CPR, and experience working with individuals with disabilities or behavioral health needs are highly valued.
