Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. The ideal candidate will possess a strong foundation in nursing practices, with experience in various clinical environments including long-term care, assisted living, hospitals, and outpatient clinics. As an LPN, you will play a vital role in delivering safe, efficient, and personalized care while supporting interdisciplinary teams and ensuring compliance with healthcare regulations. Your expertise will contribute to improving patient outcomes and enhancing overall well-being.

Duties

Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
Administer medications and injections accurately following physician orders and established protocols.
Assist with medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes.
Monitor patients' conditions closely, recognizing signs of acute pain, infection, or deterioration requiring immediate intervention.
Maintain detailed and accurate medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA regulations.
Provide compassionate patient education on medication management, disease processes such as dementia or Alzheimer's care, and post-discharge instructions.
Collaborate effectively with physicians, case managers, and other healthcare professionals to coordinate patient care.
Support patients with developmental disabilities or behavioral health needs through specialized communication and tailored interventions.

Skills

Proficiency in using EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient management.
Demonstrated experience in critical care environments including ICU or Level I trauma centers is highly desirable.
Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10).
Skilled in performing IV infusion therapy, catheter care, phlebotomy, and basic life support (BLS).
Experience working with geriatric populations in settings such as skilled nursing facilities or memory care units.
Ability to handle emergency situations effectively through first aid and airway management skills.
Familiarity with infection control protocols and aseptic techniques to ensure patient safety.
Excellent caregiver communication skills to educate patients and families about treatment plans and health coaching strategies.
