Full Job Description

JOB TITLE: Medical Care Coordinator

SUPERVISED BY: Medical Care Manager

DEPARTMENT: Programs and Services

PRIMARY RESPONSIBLITY:

The Medical Care Coordinator’s primary responsibility will be to ensure children in out-of-home care have access to health care at entry into foster care. Under general supervision of the Medical Care Manager, coordinates and performs the preparation, data entry, verification, and editing of complex, typically confidential health data.

EXAMPLE OF ACTUAL FUNCTIONS:

The person in this position must be able to perform the following essential job functions with or without reasonable accommodations:

Process and track the Initial Health Care Assessment and 30 Day Comprehensive Assessment.
Verify Medicaid benefits and assigned primary care physician.
Verify each child’s immunization status via Florida Shots and provide a copy when available to the child’s medical provider and ensure a copy is in the child’s electronic case record.
Create and submit a correctly executed medical release packet to healthcare providers within 24 hours of scheduled appointment.
Obtain placement information from full case management agency and notify and encourage caregivers to take children to their scheduled appointments.
Provide approved medical providers with an authorization to provide services for non- covered services, as needed.
Obtain medical and dental records from healthcare facilities including, but not limited to, hospitals, urgent care centers, physician offices, dental offices, Full case management agencies and other facilities that provide medical or dental care to clients.
Prepare medical and dental records to be filed in each child’s electronic case record (ASK) and submit to file room within 24 to 72 hours of receipt.
Search, retrieve and route medical and dental documents to and from the Nurse Case Management Department, Full Case Management Agencies, and health care providers.
Work with department representatives, as necessary, to facilitate and expedite the efficient flow of health care documentation, and resolve routine administrative problems.
Maintain all reports/tracking logs up-to-date on an ongoing basis.
Ensure strict confidentiality of client records according to HIPPA regulations and FCN Policies and Procedures.
Deliver a high level of customer service by effectively communicating with team members, caregivers, and healthcare providers in a timely manner.
Comply with all FCN Operating Procedures including Operating Procedures and Protocols for the Nurse Case Management Department.
Develop and maintain relationships with local healthcare providers to expand access to medical care for children in Miami-Dade and Monroe counties.
Work closely with FCN’s Intake Department to ensure all new out-of-home care removals are captured and notify pertinent parties in a timely manner.
Provides technical assistance to Full Case Management Agencies and FCN departments including, but not limited to, navigating Medicaid Managed Care System, accessing health care services, and other requests.
Act as back-up support to NCM Staff as assigned.
Assist in covering for the FCN receptionist as needed.
Participate in departmental meetings, Health Care Workgroups, CMAT Staffing(s), performance improvement activities, and other committees as assigned.
Provide data and clerical support during special department assignments. Perform other duties and complete special projects as assigned.

MINIMUM REQUIREMENTS:

High School diploma or GED with a minimum of 2 years healthcare experience, Medical Assistant experience preferred but not required.

SPECIAL SKILLS AND OTHER REQUIREMENTS:

Previous experience working in a medical setting required.
Must have comprehensive knowledge of a variety of computer software applications in word processing, spreadsheets, database and presentation software, including Word, Excel, PowerPoint and other MS Office products.
Knowledge and experience obtaining and reviewing medical/dental records.
Substantial knowledge of medical terminology.
Ability to review health records and verify patient identification.
Previous experience using electronic health records.
Strong computer skills.
Strong oral and written communication skills.
Ability to perform job function and make decisions independently.
Strong organizational skills.
Must be detail-oriented and possess the ability to work in a fast paced and high-pressure environment.
Possess the ability to work in a high paced, high pressure environment.
Ability to travel within Miami-Dade County for primary work functions.
Knowledge of social service practices and developed cultural awareness and sensitivity preferred.
English, Spanish and Creole verbal and written skills preferred.
