Full Job Description

We have openings for ****Bilingual LVN**** with experience in Primary Care to work in our Costa Mesa Clinics. These are full time positions (40 hours) with great compensation and benefits.

Adams Clinic - 2 openings / 8 hour shifts:

Job#1 - Shift is Mon - Fri 11am - 8pm with Saturday rotation 8am - 5pm
Job#2 - Shift is Mon - Fri 9:30am - 6:30pm with Saturday rotation 8am - 5pm

The pay range offered will be based on a candidates years of experience, education and skills. We are seeking LVN's with 1 - 5 years experience as a LVN in primary care.

Pay Range $30.76 - $35.75 per hour

Share Ourselves is proud to be the largest community health center in Orange County to combine wrap-around social services with health care. We currently operate several health clinics located in Costa Mesa, Santa Ana, Mission Viejo, and Newport Beach; dedicated to improving health outcomes of our community in Orange County.

JOB SUMMARY:

This position provides safe patient care by assisting Clinic Providers with treatments, procedures and clerical duties as defined in the Licensed Vocational Nurse (LVN) Scope of Practice. This position will work in conjunction with Medical Providers to triage in person and telephone patients for the clinical site. In addition the LVN Triage/Discharge Nurse will work as the liaison for ER/Hospital discharge patients.

EDUCATION/QUALIFICATIONS:

· High school diploma or GED required
· CA LVN license - Active
· Current BLS certification
· Bilingual – Spanish and English required

· Minimum of one (1) year experience as LVN in a primary care medical clinic, community health center or FQHC. Preferably 1-5 years experience.
· Minimum of one (1) year experience as a Triage LVN strongly preferred
· Electronic Health Records experience highly desired, Epic preferred.

RESPONSIBILITIES

· Manages the care of patients in collaboration with other health professionals and members of the health care team in order to make decision about treatment and attain health goals.

· Performs services requiring critical thinking as well as technical and manual skill. Performs basic nursing services, administers medication and communicates with patients with regard to the care plan and self-care requirements.

· Evaluates in person patients and telephone patients to assess patients clinically, dispense health advice, dispenses referral information, and schedule patients appropriately by indication as defined in the LVN Scope of Practice.

· Collaborates with local hospitals and patients to schedule primary care appointments for patients recently discharged from the hospital.

· Manages incoming phone calls, patient messages, and emails to ensure patients recently discharged from the hospital are contacted in a timely manner, according to SOP’s.

·On Time arrival is critical responsibility to the position.

· Other duties as assigned by Nurse Manager.
