Full Job Description

We are seeking a compassionate, organized, and dependable Social Services Assistant to join our skilled nursing facility team in Florida. The ideal candidate will have experience working in a long-term care/skilled nursing environment and hands-on experience with PointClickCare (PCC).

Key Responsibilities

Assist the Social Services Director with the day-to-day functions of the Social Services Department.
Provide support to residents and families regarding psychosocial, emotional, and adjustment needs.
Assist with new resident admissions, assessments, care planning, and discharge planning.
Maintain accurate and timely documentation in PointClickCare (PCC).
Participate in care plan meetings and communicate resident needs and concerns to the interdisciplinary team.
Assist residents and responsible parties with facility-related questions, concerns, and requests.
Help coordinate communication between residents, families, staff, physicians, and outside agencies.
Assist with resident rights, grievances, and concerns in accordance with facility policies and applicable regulations.
Maintain accurate resident records and documentation.
Assist with arranging services and resources to support residents' needs.
Promote resident dignity, independence, privacy, and quality of life.
Maintain confidentiality and comply with all applicable federal and Florida long-term care regulations.

Qualifications

Previous experience in a long-term care/skilled nursing facility required.
PointClickCare (PCC) experience required.
Previous experience in social services, healthcare, senior living, or a related setting preferred.
Strong communication, interpersonal, and organizational skills.
Compassionate and professional approach when working with residents and families.
Ability to work effectively as part of an interdisciplinary team.
Strong attention to detail and accurate documentation skills.
Ability to maintain confidentiality and handle sensitive resident information.
Knowledge of long-term care regulations and resident rights is a plus.
