NextCare Providers, LLC is an Arizona-based home health agency providing skilled nursing services to individuals and families in the comfort of their homes, including individuals receiving services through Arizona's Division of Developmental Disabilities (DDD).
Our care model is built around one-on-one, patient-centered nursing care and strong coordination between our field nurses, clinical leadership, scheduling, quality assurance, and administrative teams.
We understand that home health nurses work independently in the field, but that doesn't mean they should feel like they're on their own. Our nurses have access to an established support structure that includes:
- Director of Nursing and clinical leadership for clinical guidance and escalation
- Scheduling support for patient assignments and coverage
- Quality Assurance (QA) for documentation and compliance support
- HR and administrative support for onboarding, credentials, training, and employee needs
- Ongoing in-service education and training
- Patient-specific training and/or shadowing when appropriate
We invest in our nurses by providing the resources, communication, training, and support needed to feel confident working independently in the home health setting while maintaining high standards of patient care.
What Are Intermittent Nursing Visits?
Unlike traditional private-duty nursing or facility shifts, this position does not require remaining with one patient for an entire 8- or 12-hour shift.
Instead, you'll travel to scheduled patients within your assigned service area to complete one-hour skilled nursing visits. During each visit, you'll provide the ordered nursing services according to the patient's plan of care, complete the required documentation, and then continue to your next scheduled patient.
Depending on the patient's plan of care and physician orders, visits may include:
- Medication administration
- Insulin administration and blood glucose monitoring
- Wound care
- Catheter care
- Diabetes management
- Patient assessments
- Medication management
- Patient and family education
- Monitoring and reporting changes in condition
- Other ordered skilled nursing interventions within the LPN scope of practice

You can provide your weekend availability, and we'll work to coordinate patient assignments based on your schedule, geographic preference, and current patient needs.
Weekend Schedule
Current opportunities are available for:
Saturday | Sunday | or Both
Availability for at least two weekends per month is preferred.
Weekend assignments may include recurring patients, PRN coverage, or additional visits that become available due to scheduling needs.
The number of available visits may vary based on patient census, location, and coverage needs. Additional weekday opportunities may also become available for nurses interested in expanding their schedules.
Service Areas
Patient assignments are available throughout the Phoenix metropolitan area, including:
- West Valley
- East Valley
- Central Phoenix
Nurses are assigned based on patient needs, nurse availability, geographic location, and preferred travel radius whenever possible.
Our goal is to create practical patient assignments while maintaining continuity of care for our members.
Key Responsibilities
- Provide skilled nursing care in the patient's home according to the established plan of care and physician orders.
- Complete assigned weekend visits within required scheduling windows.
- Perform ordered nursing interventions safely and within the LPN scope of practice.
- Complete accurate and timely documentation following each visit.
- Follow Electronic Visit Verification (EVV) requirements.
- Review and follow patient-specific care instructions.
- Monitor patients and appropriately communicate changes in condition.
- Educate patients and families regarding medications, treatments, disease management, and safety when applicable.
- Communicate professionally with patients, families, scheduling, office staff, and clinical leadership.
- Promptly communicate delays, call-outs, scheduling conflicts, or inability to complete an assigned visit.
- Report patient refusals, medication concerns, incidents, missed visits, changes in condition, and other clinical concerns according to agency procedures.
- Maintain HIPAA compliance, confidentiality, professionalism, and patient dignity.
- Participate in required agency training and in-service education.
- Maintain all required licenses, certifications, and compliance credentials.
Communication & Documentation
Strong communication is especially important in home health because our nurses work independently in the field.
Nurses are expected to maintain communication regarding their patients and assignments, complete documentation within required timeframes, and promptly report concerns requiring clinical or administrative follow-up.
Our team currently utilizes:
- WellSky/Kinnser for clinical documentation
- Microsoft Teams as a primary agency communication platform
- Microsoft Outlook
- Electronic Visit Verification (EVV)
- Gusto for HR and onboarding processes
Training on agency-specific systems and workflows is provided during onboarding.
Qualifications
Required:
- Current and unrestricted Arizona LPN license
- Current CPR/BLS certification
- Valid Arizona Fingerprint Clearance Card
- Article 9 Certification – current certification preferred; candidates who do not currently hold Article 9 must be willing to obtain it before working independently with DDD members.
- Reliable transportation
- Valid driver's license
- Strong communication and documentation skills
- Ability to manage time effectively and maintain scheduled patient visits
- Ability to work independently in a home health environment
- Comfortable using technology for clinical documentation, communication, and electronic forms
Preferred:
- Previous home health nursing experience
- DDD/AHCCCS experience
- Experience working with individuals with developmental disabilities
- Intermittent nursing experience
- WellSky/Kinnser experience
- Electronic Visit Verification (EVV) experience
- Spanish or other bilingual language skills
Article 9 Certification
Because NextCare Providers LLC provides nursing services to individuals enrolled with Arizona’s Division of Developmental Disabilities (DDD), Article 9 Certification is required for this position prior to working independently with DDD members.
Don't already have Article 9? That's okay! Candidates who otherwise meet the qualifications for the position are welcome to apply and may complete the certification as part of their pre-employment requirements.
What is Article 9?
Article 9 training prepares professionals who work with individuals with developmental disabilities to understand the rights, protections, and appropriate treatment of DDD members.
The training covers areas such as:
- Rights of individuals with developmental disabilities
- Appropriate intervention strategies
- Confidentiality requirements
- Abuse and neglect
- DDD policies and procedures
- Requirements and restrictions governing the treatment and support of DDD members
DDD requires applicable provider staff to successfully complete Article 9 training before working independently with members, and the training must be provided by a DDD-approved certified Article 9 instructor.
Where Can I Complete Article 9?
One convenient option is Arizona Autism United (AZA United) Article 9 Training. AZA United currently offers instructor-led virtual Article 9 classes with advance registration.
Applicants can also use the Arizona DDD Qualified Vendor Training Resources to review official training information and the Division's list of certified Article 9 instructors/providers.

Physical Requirements
This position may require standing, bending, reaching, and assisting with patient positioning, transfers, mobility, or other physical activities associated with nursing care in the home.
Nurses must be able to safely perform the essential duties associated with their assigned patients and communicate when additional assistance or support is required.
