Full Job Description

Job Overview
We are seeking a dedicated and compassionate Residential Manager to oversee the daily operations of our residential group home. Reports to the Program Manager. Responsible and accountable for the delivery of care consistent with the philosophy, policies and procedures of Inclusive Care Community. Primary responsibilities include supervising staff members assigned to the location to ensure adequate training and resources; providing case management for individuals at the assigned location to ensure that all needs are met and documentation remains up to date; and ensuring compliance with regulatory requirements and standards of care. Remains on-call in the event of an emergency at the assigned location, including emergency response when necessary.

Responsibilities

In cooperation with ICC leadership, ensures the home operates in compliance with all regulations and requirements, including but not limited to compliance with the following: HIPAA, Medicaid HCBS guidelines, 12VAC35-105 regulations, 12VAC35-115 regulations, and agency policies and procedures.
Provides leadership by example to direct care and/or nursing staff, including acting in accordance with ICC’s mission, vision, and core values. Maintains professional conduct at all times.
Serves as a subject matter expert on best practices for direct support at the assigned service location.
Models person-centered, trauma-informed provision of services in alignment with best practices.
Guides and instructs staff in providing high quality services, including accurate documentation of services rendered to clients/residents. Training, consultation, and oversight of staff responsible for delivering services according to individual support plan.
Monitors thorough implementation and person-centeredness of all services provided at the service location.
Ensures individuals’ medical, behavioral, nutritional, and personal care needs are met in collaboration with staff members, healthcare providers, guardians and family members, support coordinators, transportation, pharmacies, and case management, including scheduling and transportation for appointments for individuals.
Completes or supervises completion of all documentation required at the service location, including but not limited to: timesheet approval, progress notes, individual service plans (ISPs), medication administration records (MARs), incident reports, quarterly reports, monthly reports, home inspections, financial ledgers, intake and output logs, skin check assessments, annual documentation, refill requests, etc.
Provides routine documented supervision for each staff member, including but not limited to monitoring training expirations, coaching, disciplinary action, punctuality and attendance, professional conduct standards, performance evaluations, etc.
Routinely audits medications, documentation, the physical environment, medical needs, training needs, primary records, and individual funds on site.
Plans and implements in-service education programs and staff meetings.
Accurate and transparent communication with ICC leadership, including ensuring that issues or concerns are properly addressed. Actively seeks guidance when needed.
Participation and/or organization of required meetings, including internal meetings, annual meetings, SIS meetings, staff meetings, etc.
Oversees emergency procedures at the service location, including auditing emergency supplies on hand, updating emergency plans, monthly completion of fire drills, auditing first aid kits, and overseeing staff training on emergency procedures.
Takes an active role in each individual’s circle of support, maintaining positive relationships with other team members for each individual.
Monitors inventory and resources in the group home, including expiration dates. Orders additional food or supplies as needed.
Uses and trains employees on utilization of required software and devices, including but not limited to: company email, HR information system, electronic health record, electronic medication administration record, electronic filing system, fax and scanner, etc.
Serves as the primary communication touchpoint for members of individuals’ circle of support, including guardians, family members, support coordinators, day supports, and physicians.
Manages timely completion of all required documentation, including procuring third-party signatures as required.
Accurately reports key performance indicators (KPIs) as required.
Provides direct care for individuals at the service location as needed.
Able to effectively perform all job duties of DSPs and/or LPNs in the home as applicable if urgent coverage needs arise.
Remains on-call for emergency situations, including in-person incident response and response to urgent or last-minute coverage needs,
Other duties as assigned.

Skills

Minimum of two years’ experience in public health nursing, hospital nursing, DD waiver services, mental health services, IDD services, behavioral therapy, or IDD case management.
Two years of supervisory experience is strongly preferred.
Knowledgeable of Medicaid regulations pertaining to Medicaid HCBS guidelines is strongly preferred.
Ability to communicate effectively both orally and in writing.
Ability to work independently, while maintaining open communication with ICC leadership.
Minimum two years’ experience working with the IDD community.
Excellent oral and writing skills.
Working knowledge of the Medicaid Waiver programs.
Possession of satisfactory driving record and reliable transportation. Ability to operate agency vehicles.
Proficient computer skills including Microsoft Word and Excel.
Ability to lift, push, and/or pull up to 50lbs.
Maintains current training and certifications as required, including but not limited to: CPR, Crisis Wave, Human Rights, HCBS, LPN licensure if applicable, etc.
